Excerpt from The Archives of Pediatrics, Vol. 4: A Monthly Journal Devoted to the Diseases of Infants and Young Children; January to December, 1887 The cases which form the basis of this paper came under the writer's observation within a period of eighteen months; a fact which has led him to the conclusion that primary renal disease in infancy is by no means so rare as has generally been supposed. They are published for the purpose of drawing more attention to the importance of the examination of the urine at this time of life, something which heretofore has been overlooked almost entirely except in cases of diphtheria and scarlet fever. Case I. Acute interstitial nephritis in a child of seven months; three weeks duration; steadily high temperature; death; autopsy. - W. D., male, inmate of New York Infant Asylum, was taken, without apparent cause, quite suddenly ill, January 16, 1886, with high fever and severe accompanying symptoms. The family history was tubercular on the father's side. With the exception of a few days, six weeks before, while teething, the child had never been sick. At noon, on the date referred to, the temperature was found 104 F.; pulse, 160; respirations, 40. No evidence of disease in the chest or elsewhere was found, and six grains of antipyrine were given in three doses within an hour. Excerpt from Archives of Pediatrics, Vol. 18: A Monthly Journal Devoted to the Diseases of Infants and Children; January to December, 1901 The subject of this record, a girl, was born August 14, 1899. At birth she was smaller than the average, but well developed, of an estimated weight of six pounds, and thought by the mother to be about two weeks premature. The breast was readily taken every six hours the first day and every four hours the second. Boiled water was given every two hours when the child was not nursed. The mother's temperature did not exceed 100 F. At any time and her bowels were moved by a laxative on the second day. The third day the milk flow was established abundantly, but it was reported that the baby vomited freely in from five to twenty minutes after each nursing, bringing up scarcely altered milk and some mucus. The nurse thought the baby had spit up a little Since birth. During the previous night the baby had been blue and cold, which conditions had been overcome with hot bottles. Of fat gave a top milk containing an average of per cent. Of fat. It will be seen that the fat in the ten bottles of February lgth varied from 4 per cent. To per cent., and the fat in the top 8 ounces varied from 16 to 20 per cent. Incidentally it will be observed that the 4 per cent. Milk gave about 16 per cent. Of fat in the top 8 ounces, which is 2 per cent. Higher than I have heretofore found it to be, and higher than it has generally been stated to be. This means that we have been feeding our infants with a larger percentage of fat than we have intended to, or thought we were feeding. The effect of this is that when we thought we were feeding per cent. Of fat we were actually giving from to per cent. Of fat. When we thought we were giving 3. 5 per cent. We were actually giving from per cent. To per cent. The dif ference between and 5 per cent. Fat in a baby's food is a matter of no small importance, and probably accounts for some troubles that we have seen on certain days and which we could not explain. The chart shows that the variation in the same day from the same farm is from 1 to per cent. Of fat in the whole milk. If the amount were uniform from day to day, we could adjust it to the needs of the infant; but when we use 15 per cent. Top milk on one day and 20 per cent. Top milk the next, serious disturbance may result. There is manifestly a necessity for some means of securing a more uniform milk than these figures show. The cause of the variation may be in the methods used in bottling, or it may be in the breed of the cows in the stable.
