For 30 years, we have taught limb correction surgery at the annual Baltimore Limb Deformity Course (www.DeformityCourse.com). We have used these hands-on experiences with our students to guide us when creating this book. The Art of Limb Alignment with Excerpts from the Baltimore Limb Deformity Course Workbook is a concise guide for beginners starting on their journey to master the art of limb alignment. Understanding limb alignment and malalignment is a critical task for surgeons who treat lower extremity deformities. The first task is to characterize the deformity: Is the bone angulated? In what direction is the bone angulated? What is the level of the deformity? Where is the apex of the deformity? What is the magnitude of the deformity? Is the bone short? Is it rotated on its axis? All these questions must be answered before correcting the problem.In Chapter 5 of this ninth edition, we have developed new approaches to analyze sagittal plane deformities. The sagittal plane has always been challenging to teach because the range of motion of the knee adds a level of complexity. Joint motion or lack of joint motion contributes to or compensates for sagittal plane deformities. Without understanding and quantifying the soft-tissue contribution, the deformity will not be fully addressed and corrected. We have developed two new angles to analyze the confounding factor of knee motion in the sagittal plane: the sagittal mechanical axes angle (SMAA) and the sagittal joint line angle (SJLA). We also developed three different ways to calculate the soft-tissue contribution: the Seesaw Method, the Formula Method, and the Table Method. We decided to present three options so that surgeons can select the method that best suits their learning style (from more visual to more mathematical). Using one of these three methods to carefully evaluate the soft-tissue contribution will allow for an accurate assessment and the creation of a successful treatment strategy.This edition also includes excerpts from the Baltimore Limb Deformity Course Workbook (starting on page 243). The workbook is an essential part of the planning workshops that will be offered during the Virtual 30th Annual Baltimore Limb Deformity Course (www.DeformityCourse.com) that will be held in August 2020. In Chapter 10 of this tenth edition, digital planning software (Bone Ninja app, available as an iPad app teaching tool through the App Store) is used to show the reverse planning method for femoral deformity. Reverse planning using manual methods was introduced by Rainer Baumgart in 2009. In a normal femur, the anatomic and mechanical axes deviate from each other by 5˚ to 7˚. By using reverse planning to "begin with the end in mind," the surgeon will minimize risk of unwanted mechanical axis deviation secondary this anatomic-mechanical axis divergence. In Chapter 13, the principles of MAP the ABCs are applied to common conditions seen in foot and ankle deformities. Foot and ankle deformity correction can be challenging to treat due to the compensation from the joints within the foot. This chapter shows step-by-step analysis and correction of four deformities: a pes planus foot with hallux valgus, cavus foot, rockerbottom midfoot, and ankle varus malunion. Each year in the United States, an estimated 40,000 persons lose a limb. Of these amputees, approximately 30% lose a hand or an arm. This loss is most frequently related to trauma occurring in the healthy young adult male and is often work related. Approximately 3% of all amputees are born with congenital limb absence. In children, the ratio of congenital to acquired amputation is 2: 1, and the ratio of upper-limb to lower-limb amputees is 1. 2: 1. Therefore, since relatively few amputations result in upper-limb loss, only a small number of health practitioners, even those specializing in amputee rehabilitation, have the opportunity to provide services for a significant number of arm amputees. As a result, clinicians need to share their experiences so that the full range of options for optimum care and rehabilitation of the patient population may be considered. To meet this challenge for wider communication of clinical experience, a group of upper-limb amputee specialists met in Houston, Texas, in 1981 to serve as the core faculty for a course entitled "Contemporary Issues in Upper Extremity Amputation and Prosthetic Function. " This program provided the opportunity for surgeons, physiatrists, engineers, prosthetists, social workers, psychologists, occupational therapists, and physical therapists from the United States and Canada to discuss their extensive experience in working with upper extremity amputees. A second conference continuing the discussion of upper limb amputee rehabilitation was held one year later.
