Since Picketts failed charge at Gettysburg, the frontal infantry assault had been known as obsolete. Nevertheless fifty years later, Allied military leaders in the Great War persisted in using it as a military tactic. Italian military leaders were no exception not even accepting the deadly effect of machine guns or quick-firing artillery. The Battles of the Isonzo on the Austro-Italian Front have now been classified with Verdun as to intensity and casualty lists. Mountain warfare on the Isonzo River Valley resulted in almost two million casualties from avalanches, frostbite, malaria, cholera, as well as prisoner-of-war starvation. Using the attacco frontale the blood of the illiterate fanti was used as coin to purchase terrain pushing the enemy back leading to Vienna's request to Berlin for help, leading to Caporetto.

Medical futility is a controversial issue not only in its definition but also in its application. There are few books on the subject, and those in existence mostly focus on the situation in the United States. This title, however, provides extensive international perspectives on medical futility. This book will benefit healthcare professionals as well as health policy makers around the world. It allows them to see how different countries approach the issue of medical futility and their experiences in dealing with this issue. The complexity of the issue, and in particular how some countries innovatively address it in an ethically sound manner, is clearly presented. Contents:So-Called Futile Care: The Experience of the United States (Robert M Veatch)The Reality of Medical Futility (Dysthanasia) in Brazil (Leo Pessini and William Saad Hossne)Medical Futility and End-of-Life Issues in Belgium (Jan L Bernheim, Thierry Vansweevelt and Lieven Annemans)The Concept of Medical Futility in Venezuela (Gabriel d'Empaire)Medical Futility in the Russian Federation (Olga I Kubar, Galina L Mikirtichian and Marina I Petrova)Medical Futility in Australia (Dominique Martin)Medical Futility in Japan (Yasuhiro Kadooka and Atsushi Asai)Medical Futility in China: Ethical Issues and Policy (Yongxing Shi, Mingjie Zhao, Yang Yang, Cunfang Mao, Hui Zhu and Qingli Hu)Medical Futility in Korea (Ivo Kwon)Medical Futility from the Swiss Perspective (Tanja Krones and Settimio Monteverde)Medical Futility in Turkey (Berna Arda and Ahmet Aciduman)Medical Futility in the United Arab Emirates (Said Abuhasna and Ali Abdulkareem Al Obaidli)Medical Futility in Iran (Alireza Bagheri) Readership: Healthcare professionals, health policy makers, physicians and nurses, lawyers academics, researchers, graduate students and lay public. Keywords:Medical Futility;Bioethics;End of Life;Clinical Ethics;Healthcare SystemsKey Features:Each chapter discusses medical futility in a particular country, including the US, China, Australia, Switzerland, Brazil, Korea, Japan, Iran, United Arab Emirates, Venezuela, Russia, Italy, and TurkeyPresents experiences and challenges relating to medical futility in several countries with different cultural and economic backgroundsProvides information about the topic and elaborates on healthcare systems, payment and insurance systems as well as end-of-life issues in contributing countriesReviews: “The topic of ‘futility’ is at once important and controversial. Important because it deals with a critical element of end-of-life care, and yet controversial because of disagreements about its meaning and use. This superb collection of papers deals well and fully with those elements, and in a rich global context.” Daniel Callahan President Emeritus of the Hastings Center “As medical costs continue to escalate internationally and technology offers more and more chances of prolonging the length of life — but not necessarily its quality — it is vital that we think more clearly and systematically about what can be justifiably described as ‘medically futile’. Dr Bagheri's edited volume is a major contribution to this highly topical subject, and one of its great strengths is its international scope, with scholarly contributors from a wide range of countries in Europe, the Americas, Australia and Asia. The book should become a standard text for all courses in healthcare ethics.” Alastair V Campbell Chen Su Lan Centennial Professor in Medical Ethics National University of Singapore “Medical futility — a term that is often used but seldom clearly understood — is the quintessential bioethics topic, combining as it does philosophical questions about the ends of medicine, central issues in physician-patient and patient-family relations, and controversies about societal obligations to continue care that can extend life but not restore functioning. It is thus perfect for the sort of comparative examination that this cross-national volume so richly provides. The chapters are individually fascinating, while collectively illuminating how historical, cultural, economic, political and philosophical differences make medical futility such a rich subject for study and such a difficult issue to resolve clinically.” Alexander M Capron University Professor, University of Southern California; former Director of Ethics, Trade, Human Rights and Health Law, World Health Organization “Bagheri and his colleagues give a fascinating account of change in concepts of medical futility and allied concepts in a world of globalization and knowledge sharing … and teach us that global exchanges of diverse cultural, religious, and socioeconomic beliefs and factors are invigorating both intranational and international debate over what constitutes medical futility, slowly but surely leading to changes in medical practices and policies that reflect the global pluralism of our age.” The American Journal of Bioethics

Forgiveness is important in international politics because it can save thousands of lives. Its opposite, vengefulness, has played a significant part in various wars of the 20th and 21st centuries. These conflicts are examined in this book, showing how forgiveness could have avoided the tremendous ensuing bloodshed. Despite its importance, in the context of international relations, forgiveness as a means of preventing the outbreak of war (as opposed to facilitating reconciliation after conflicts) has largely been neglected as a subject of study. Indeed, it has also been ignored by politicians, as a result of which there are few examples of forgiveness to study compared with those of revenge. This book reflects this reality, but also seeks to change it by raising public awareness of the importance of forgiveness in international affairs and the need to demand that political leaders explore this avenue. The book also provides a succinct, informative guide to the background of today’s international affairs. Each chapter can be read independently and highlights either forgiveness in action or the futility and loss of life caused by vengefulness, demonstrating where and how forgiveness could have made a dramatic difference.

Pannenberg on Evil, Love and God examines a much-neglected aspect of the theological thought of one of the most original contemporary German theologians, Wolfhart Pannenberg: his theological and philosophical understanding of evil and its relationship to the love of God. The book seeks to correct a widely held misconception that in his theology, Pannenberg has neglected the darker side of the world, concentrating instead on an optimistic picture of the future. This book argues that questions of evil hold a central place throughout Pannenberg’s writing and seeks to draw out the implications of his wrestling with these issues. The Introduction sets the scene by considering the nature of the question of evil and argues that a theological response must be made as part of a global view of the world and not in isolation from other themes. The succeeding chapters develop this theme through a reading of Pannenberg’s theology.

Futility, or The Wreck of the Titan is Morgan Robertson’s 1898 novella about the unsinkable ship Titan, which goes down after striking an iceberg in the North Atlantic. Disgraced former naval lieutenant John Rowland is working as a deckhand on the Titan when it strikes an iceberg and capsizes. Saving the younger daughter of a former lover by jumping onto the iceberg with her, Rowland and his charge are eventually rescued and return to their homes. Written before the Titanic was designed, Futility, or The Wreck of the Titan’s narrative is eerily similar to the real-life disaster that occurred the night of April 15, 1912: both Titanic and Titan were considered to be unsinkable; neither ship carried enough lifeboats for the number of passengers on board; and both sank after striking icebergs. The sinking of the Titanic in the frigid waters of the north Atlantic in the early hours of April 15, 1912, is arguably the most famous peacetime maritime disaster of modern history, and one that continues to capture popular imagination a century later. Hybrid Frequentist/Bayesian Power and Bayesian Power in Planning Clinical Trials provides a practical introduction to unconditional approaches to planning randomised clinical trials, particularly aimed at drug development in the pharmaceutical industry. This book is aimed at providing guidance to practitioners in using average power, assurance and related concepts. This book brings together recent research and sets them in a consistent framework and provides a fresh insight into how such methods can be used. Features: A focus on normal theory linking average power, expected power, predictive power, assurance, conditional Bayesian power and Bayesian power. Extensions of the concepts to binomial, and time-to-event outcomes and non-inferiority trials An investigation into the upper bound on average power, assurance and Bayesian power based on the prior probability of a positive treatment effect Application of assurance to a series of trials in a development program and an introduction of the assurance of an individual trial conditional on the positive outcome of an earlier trial in the program, or to the successful outcome of an interim analysis Prior distribution of power and sample size Extension of the basic approach to proof-of-concept trials with dual success criteria Investigation of the connection between conditional and predictive power at an interim analysis and power and assurance Introduction of the idea of surety in sample sizing of clinical trials based on the width of the confidence intervals for the treatment effect, and an unconditional version.
