Misused, abused, diverted, and counterfeited, benzodiazepines have serious potential for substance use disorder, and are among the leading causes of drug-related overdose deaths. The story of the benzodiazepines is a fascinating one: the synthesis of a long series of inert compounds and the abandonment of the project: months later, the renewal of the project and the discovery that this compound R05-0690 (Librium ®) was a sedative and muscle relaxant in laboratory animals: the recognition that its postulated structure was wrong and that it was in fact a member of an entirely new chemical class: the excitement caused by the discovery of the powerful taming effect of the drug in wild animals: the even greater excitement in medical circles when its dramatic anxiety relieving effects were cstablished in humans: the subsequent enormous world wide usc of drugs of the benzodiazepine group. I have reason to know the story well for during that period I worked in various capacities in research, marketing and manage ment in the British subsidiary of F. Hoffmann-Ia Roche. It was during this period that I became interested in whether dependence on benzodiazepines could occur and if so to what extent, but felt that this could be examined best from outside the concern. Having now retired from Roche back to academic medicine I have taken the opportunity to examine and analyse the evidence.

This review, deputizing for many others, reflects the friendly reception enjoyed by the first volume of Benzodiazepines, which was published in 1982 and apparently closed a gap in the ben zodiazepine literature. In the meantime, Benzodiazepines has established itself as a standard book, as evidenced by numerous letters and quotations. Suggestions were also soon made for a new edition in view of the unusually rapid development in the field of the benzodiazepines. It became quickly obvious, however, that it would not be sufficient to publish a revised second edition, but that a completely new second volume would be required for which, however, the successful previous format could be largely retained. The following considerations seem worth mentioning in connection with the preparation of Volume II: - To ensure continuity with Volume I as far as possible, the list of references was consecutively numbered (references 1 to 3779 in Volume I, references 3780 to 11338 in Volume II). Whereas in Vol. I the substances appear in the sequential order of their historical development they are listed in alphabetical order in Vol. II.

To comment at length on the importance of the benzodiazepines seems superfluous within the scope of this preface. No other class of active substances has experienced an even approximately compa rable advance in the past two decades. It is therefore not surprising that the formerly dominant barbiturates and bromocarbamides have had to give way in many fields to the benzodiazepines, which now rank first (Proudfoot and Park [1828]). Closely linked with the great therapeutic importance of the benzodia zepines are analytical problems. Detection and the determination of blood levels can be necessary under therapeutic aspects, for instance in working out optimally effective levels in the treatment of epileptic conditions ("drug monitoring"), but also in connection with questions at issue in toxicology and traffic medicine. A toxicological analysis can be subdivided into the following steps: Detection (identification including screening) Determination (e. g. blood, plasma or serum levels) Interpretation of the analytical results. This book is intended as a contribution to each of these chapters: The part "Analytical Data" (pp.I-122) gives a comprehensive collec tion of data, e.g. general and chromatographic data (TLC, GLC) as well as spectra (UV, IR, MS) of 19 commercial preparations, 23 important metabolites and 18 hydrolysis derivatives. Information about biotransformation and the possible formation of aminobenzo phenone derivatives is also given. The most important analytical methods are presented in an extensive review on pp.123-204 in order to make it possible to select the optimum method on the basis of the essential data.
