With "accountability" fast becoming the byword of the age, psychotherapists are challenged to demonstrate both the therapeutic and cost effectiveness of their interventions. While traditionally these two goals have been considered mutually exclusive, new research on the briefer therapies appears to show that less can sometimes be more. Fewer sessions of a carefully planned, focused and well-executed short-term intervention may contribute more to positive change that protracted analysis. Forms of Brief Therapy is the first volume to comprehensively and critically address the issue of how the element of time can be used as a therapeutic tool in psychodynamic and behavioral systems, and across individual, couple, family and group interventions. The authors put to rest the pervasive myth that there is an ideal, specific duration for most therapy. Instead, they offer a variety of approaches that combine careful examination and integration of presenting problems and patient characteristics. At times, evaluation may indicate as many as fifty sessions. In other cases, as evidenced with special force in Bloom's provocative presentation, just a single focused session may suffice. As Weiss and Jacobson put it in their marital therapy contribution, the purpose of this book is to structure therapy that is brief by design rather than by default. The contributors to Forms of Brief Therapy are among the most renowned practitioners and thinkers in their respective fields. Their critical overviews of research, and pragmatic analyses of therapy design, will prove highly instructive to clinical researchers and teachers who wish to better understand the relationship of therapeutic structure to underlying mechanisms of change. Mental health practitioners will find this volume eminently useful in their daily work. It provides explicit recommendations for patient selection, evaluation, and choice of therapeutic techniques which are richly illustrated by case materials. Unexcelled as clinical guide and reference, Forms of Brief Therapy is also ideally suited as a textbook for psychotherapy courses.

Following the publication of the Handbook of the Brief Psychotherapies (Wells & Giannetti, 1990), the editors began to conceptualize the idea of a collection of case studies encompassing a number of the commonly en countered clinical problems that have been treated with such ap proaches. The Casebook of the Brief Psychotherapies is the result. The Case book details clinical interventions with client populations as diverse as substance abusers, torture victims, the physically handicapped and other exceptional groups, and the economically disadvantaged with emotional and behavioral problems, as well as individuals experiencing sexual dysfunction or eating disorders. In addition, topics such as be reavement, depression, anger, and many crucial aspects of marital and family therapy are discussed by eminent clinical practitioners. Although the cases draw heavily upon cognitive behavioral and strategic structural formulations, psychodynamic, interpersonal, and experiential ap proaches are also included. The Casebook is clinically oriented, with a minimum of theory. Am ple case material and commentary allow the reader to experience direct ly the application of brief therapy to specific client problems. What emerges from this compendium of approaches and problems is a tap estry of action-oriented, problem-solving, skill-building, rational ap proaches to therapy that balance the client's ability to change with the demands and limits of time.

In today's world of managed care -- characterized by limited mental health resources, emphasis on accountability, concerns of third-party payers, and consumer need -- the demand for mental health professionals to use briefer therapeutic approaches is on the rise. Fully 84% of all clinicians are doing some form of planned brief therapy (6-20 sessions per year per patient). Yet despite clinical advances and outcome data that demonstrate the effectiveness of short-term therapy, many therapists -- in fact, 90% of those whose theoretical orientation is psychodynamic rather than cognitive-behavioral -- are reluctant to learn briefer interventions, seeing value only in long-term, depth-oriented work. The second edition of this Concise Guide is intended to help educate both beginning and experienced clinicians in the strategies and techniques of time-attentive models and to foster more positive and optimistic attitudes toward using these important therapies. The seven therapeutic models presented here -- including an entirely new chapter on time-limited group therapy -- highlight the importance of the interpersonal perspective. The seven models, one per chapter, represent well-established short-term approaches to clinical issues that therapists commonly encounter in their clinical practices. These models also have clearly defined intervention techniques and formulation strategies and can be used within the 10- to 20-session time frame of most managed care settings. The first part of each chapter dealing with a therapeutic model lists the various presenting problems the authors deem most suitable for treatment by that particular approach. The authors discuss the overall framework of each model, selection criteria, goals, therapeutic tasks and strategies, empirical support, and relevance for managed care, with clinical cases to illustrate the application of each model. The authors include updated chapters on supportive, time-limited, and interpersonal therapies; time-limited dynamic psychotherapy; short-term dynamic therapy for patients with posttraumatic stress disorder; brief dynamic therapy for patients with substance abuse disorders; an entirely new chapter on time-limited group therapy; and a final chapter on the reciprocal relationship between pharmacotherapy and psychotherapy. Meant to complement the more detailed information found in lengthier psychiatric texts, this Concise Guide (it is designed to fit into a jacket or lab coat pocket) is a practical and convenient reference for psychiatrists, psychiatric residents, and medical students working in a variety of treatment settings, such as inpatient psychiatry units, outpatient clinics, consultation-liaison services, and private offices.

The last two decades have seen unprecedented increases in health care costs and, at the same time, encouraging progress in psychotherapy research. On the one hand, accountability, cost-effectiveness, and efficiency have now become commonplace terms for providers of mental health services whereas, on the other hand, an increasingly voluminous literature has emerged supporting the effectiveness of a number of types of psychotherapies. There now exists the possibility for the design and delivery of mental health services that-drawing upon this literature-more closely approximate empirically established data concerning the appropriateness and effectiveness of psychotherapy. The Handbook of the Brief Psychotherapies is intended to capture one major thrust of this movement: the development of a group of empirically grounded, time-limited therapies all sharing a common interest in the clinical utilization of a structured focus and an emphasis on time and action. For many years, professional self-interest, competing theoretical para digms, and the vagaries of practice, wisdom, and clinical myth have influenced the practice of psychotherapy. A critical questioning of the resulting, predomi nantly nondirective, open-ended, and global therapies has led to a growing emphasis on action-oriented, problem-focused, time-limited therapies. Yet, ironically, this interest in the brief psychotherapies has not so much involved a radical departure from traditional therapeutic modalities as it has emphasized a new pragmatism about how time, action, and structure operate in life as well as in therapy.

The Role of Brief Therapy in Attachment Disorders provides a comprehensive summary of the range of approaches that exist within the brief therapy world, including Cognitive Analytic Therapy, Cognitive Behavioural Therapy, Eye Movement Desensitisation and Reprocessing, Ericksonian Therapy, Neurolinguistic Psychotherapy, Provocative Therapy, Rational Emotive Behaviour Therapy, and Self Relations Therapy. Historically, many of the founders of these therapies commenced their psychotherapy careers as psychodynamic or systemic therapists, and have changed their allegiance to briefer therapies, viewing these as more respectful and offering greater potential for assisting the client to change through an outcome-oriented approach.
