"This is Miss Jewett's first novel, her former efforts having been confined to short stories. To a plot of unusual interest she brings, as a physician's daughter, a close familiarity with the incidents of a doctor's life; and this, combined with wonderful acuteness of observation and a graceful styled, make a book of very unusual interest. 'I've come across some stories in my time. The things people tell you when they think they're on the brink...' A country doctor recounts a story to an aspiring Russian novelist. The tale is unexpected, short and bittersweet, rather like its subject: a love affair between the doctor and his dying patient, the beautiful and cultured Alexandra Andreyevna. Thrown together by her condition, they find a love imbued with an honesty and an urgency that most would find unbearable. But this young woman's life is particularly fragile and in his desperate bid to cure her the doctor unwittingly prescribes the most dangerous drug of all. The Country Doctor was first published in the Russian literary magazine The Contemporary in the late 1840s. It was one of many tales which would later comprise The Sportsman's Notebook. Simon Day dramatises this enchanting story of frustrated love, bringing the elegance of Turgenev's prose to life in a new way.

An anthology that addresses the changing nature of rural medicine in the United States "These authors courageously document the emotional and literally physical vulnerabilities they experience while delivering care in rural communities. ... This book exquisitely illustrates the complexity of 'dual relationships' and boundary issues in rural practice."--Family Medicine Over the past thirty years, rural health care in the United States has changed dramatically. The stereotypical white-haired doctor with his black bag of instruments and his predominantly white, small-town clientele has imploded: the global age has reached rural America. Independently owned clinics have given way to a massive system of hospitals; new technology now brings specialists right to the patient's bedside; and an increasingly diverse clientele has sparked the need for doctors and nurses with an equally diverse assortment of skills. The Country Doctor Revisited is a fascinating collection of essays, poems, and short stories written by rural health care professionals on the experiences of doctors and nurses practicing medicine in rural environments, such as farms, reservations, and migrant camps. The pieces explore the benefits and burdens of new technology, the dilemmas in making ethically sound decisions, and the trials of caring for patients in a broken system. Alternately compelling, thought provoking, and moving, they speak of the diversity of rural health care providers, the range of patients served in rural communities, the variety of settings that comprise the rural United States, and the resources and challenges health care providers and patients face today.

A humorous, anecdotal history of the author's medical practice from 1936 to 1988, it describes a simpler time, during which doctors practiced without many of the problems that face modern physicians: forms to complete for every procedure, overcrowded offices, second opinions, high insurance costs, etc. In fact, much of the book is about the unique doctor-patient relationships that existed before the age of health insurance. It was a different kind of time, when the family doctor often met his patient at the emergency room and remained the sole treating physician throughout the hospital stay.

Though not as well-known as the writers she influenced, Sarah Orne Jewett nevertheless remains one of the most important American novelists of the late nineteenth century. Published in 1884, Jewett’s first novel, A Country Doctor, is a luminous portrayal of rural Maine and a semiautobiographical look at her world. In it, Nan’s struggle to choose between marriage and a career as a doctor, between the confining life of a small town and a self-directed one as a professional, mirrors Jewett’s own conflicts as well as eloquently giving voice to the leading women’s issues of her time. Perhaps even more important, Jewett’s perfect details about wild flowers and seaside wharfs, farm women knitting by the fireside and sailors going upriver to meet the moonlight, convey a realism that has seldom been surpassed and stamp her writing with her signature style. A contemporary and friend of Willa Cather, Harriet Beecher Stowe, and Julia Ward Howe, Sarah Orne Jewett is widely recognized as a pathfinder in American literary history, courageously pursuing a road less traveled that led the way for other women to follow.

This book contains my reflections upon my 26 years as an "old time" generalist family physician in rural America. It is merely a collection of stories that depict some of my many experiences over the years. The stories mirror those of many of my generalist colleagues who have experienced "the trenches" of rural primary care. My stories span the breadth and depth of the discipline of rural family medicine. Stories range from assisting mothers giving birth to caring for and comforting those at the end of life, from performing surgery to becoming involved in one's community, and from making house calls to providing emergency room care. Some are silly, some are sad, some are funny, and some may make you mad. JIMMIE ASHCRAFT, M.D. Doctor Ashcraft is a graduate of the University of Oregon Medical School. He was a member of the clinical staff of the University of Washington Medical School for 30 years. Doctor Ashcraft and his wife, Kay, live in Montana. They have three grown children and six grandchildren.
