This volume explores the nature of health and health-care experiences in Russia by comparing societies and communities with different socio-cultural conditions. The unique use of longitudinal data collected over ten years, allows the authors to address key questions on Russians individual experiences of health care and their understanding of its influencing factors. They explore the methods of self treatment and illness prevention in combination with the effects poverty and treatment availability can have on the standards of living for the people surveyed. This pertinent issue follows a time of rapidly worsening health status amongst the Russian population and a grave decline in male life expectancy. The findings are set within the context of experience from Finland and the UK, allowing the authors to explore the challenge of the Russian health-care crisis to Western European models of health status and health care.

Health capital in Russia is in steep decline. Today the Russian population is decreasing by more than 700,000 per annum. Life expectancy has decreased significantly since it peaked in the mid-1960s. Infectious diseases, including an emerging HIV/AIDS epidemic, are threatening to worsen Russia's health crisis and further overwhelm a dilapidated healthcare system. Soviet and Russian government policies aimed at preserving health capital have failed consistently. Government policies and intervention have contributed to the crisis. The purpose of this research was to determine a possible explanation for the continuity in ineffective government policy. The analysis indicates the influence of a paternalistic political culture permeates the political process. As a result, the government is free to pursue its own agenda without a significant degree of accountability to the population. Issues affecting health capital are not a priority of the government. The consequence, therefore, is short-sighted and uncoordinated government policy and programs that are underfunded. Long-term improvements to Russia's health capital will require a shift in the political culture. State-society relations must evolve to allow and encourage greater interaction between state officials and the general population. Without government accountability or inidividual responsibility, health capital in Russia will continue to decline.

Murrugarra and Signoret analyze the widespread effects of the financial crisis in Russia to explore the vulnerabilities of households in Moldova. They show that the crisis had differential impacts on households, affecting most the urban and better-off. Households' decisions about education and health resulted in decreased utilization and expenditures. The enrollment of young children from better-off households did not improve while others did. Secondary school enrollment of children from better-off households decreased after the crisis, in part because of the need to release labor supply. Health utilization decreased mainly for primary health care (not for hospitals), both for better-off households and in rural areas. Some of these changes are due to limited household resources (health), decreased public spending (health and education) or the need to increase households' labor supply (education of teenagers). Social benefits played a very limited role in mitigating these effects, solely in health care use. Households' assets helped to offset some of the negative effects of declining incomes.This paper - a product of the Human Development Sector Unit, Europe and Central Asia Region - is part of a larger effort in the region to understand the effects of crises on human development.
