In the last few years, the primary diagnosis of soft tissue tumours has emerged as an important new target for fine needle aspiration (FNA). The purpose of this book is to facilitate the cytological evaluation of FNA smears from soft tissue tumours and to suggest cytological criteria for histotype diagnoses. Its primary aim is to describe and illustrate not only the most common types but also those rare tumours where cytological features have been largely described in case reports and in small series. The diagnostic use of ancillary methods is discussed and illustrated, and, in order to facilitate the diagnostic work-up in FNA samples, smears are also classified according to their principal microscopic patterns. The selection of entities presented, their diagnostic features and differential diagnostic considerations are mainly based on the experience with FNA in the primary diagnosis of soft tissue tumours in patients referred to the Musculoskeletal Tumour Centre at the University Hospital in Lund, Sweden over the last 30 years. The illustrations have mainly been culled from cases in the files of the Department of Pathology and Cytology, Lund University Hospital. Soft tissue tumours are relatively rare and often difficult to correctly diagnose in fine needle aspirates. Therefore, this publication should be welcomed by all cytopathologists involved in the FNA diagnosis of tumours as a valuable addition to their benchside textbooks.

Cytopathology of Bone and Soft Tissue Tumors provides the practicing pathologist with a single reference for describing and illustrating the cytologic features of musculoskeletal tumors. Using fine-needle aspiration (FNA), the approach of this work is encyclopedic: both relatively common and relatively rare lesions are depicted. It is expected that Cytopathology of Bone and Soft Tissue Tumors will serve to widen the usage of the FNA technique which can: substantially decrease patient morbidity; lessen the complications that arise due to other biopsy techniques; and shorten the time as well as expense required for diagnosis. The chapters on soft tissue lesions are organized by direction of differentiation. Each chapter documents both the common and uncommon lesions within the tissue group. Summaries of clinical findings are given along with the histopathologic and cytologic description. Key diagnostic points (in tabular form) and a discussion of the differential diagnosis complete each section. The section on skeletal lesions is organized along predominant cell type seen in smears. This approach facilitates grouping of lesions into diagnostically useful categories, allowing the pathologist faced with an unfamiliar lesion to rapidly access the portion of the text most useful for differential diagnosis. Within each chapter, the organization is similar to that within the soft tissue chapters. Soft tissue tumors are a very heterogeneous group of tumors in terms of histogenesis, morphology, cytogenetics, molecular biology, clinical manifestation, and prognosis. Their spectrum is fascinating for morphologists and basic scientists alike. Yet precisely this variability in the morphologic manifestation of soft tissue tumors, specifically their histologic and cytologic patterns, presents great difficulties to any effort to categorize them. Although many soft tissue tumors are today defined not only by histology but also by immunohistochemical, cytogenetic, and molecular biological findings, the histogenesis of many soft tissue tumors, in particular malignant ones, continues to be unknown. This is associated with the fact that the actual precursor cells that lead to these tumors have frequently not yet been identified. For this reason, the customary classification of malignant soft tissue tumors is primarily not histogenetic, but actually according to the dominant phenotype, however characteristic it is. Of course, an exact morphologic examination of soft tissue tumors con tinues to be an essential prerequisite for making a diagnosis and determining a therapy. The use of a wide range of additional modern examination techniques, however, can make a substantial contribution toward more precisely defining the biological behavior of a tumor, which without doubt can have therapeutic implications.
