Family functioning has been an essential aspect the management of chronic illnesses in children. The way the family adapts to the child’s illness has shown to impact how the illness progresses, adherence to treatment, mental health, and social problems (Brown & Lambert, 1999; Cipolletta et al., 2015; Herzer et al., 2010). Furthermore, the family’s adaptability can be influenced by how the family and child perceived the severity of their illness. These impacting the family’s problem-solving skills and communication, forward affecting their quality of life (Felnhofer et al., 2019). A sample of 13 Puerto Rican caregivers of children with asthma or diabetes completed a survey to assess how illness perception and quality of life impact family functioning and the overall management of asthma or diabetes in children. The survey was comprised of the World Health Organization Quality of Life, Brief Illness Perception Questionnaire and the McMaster Family Assessment Device. Due to the limitation of the sample, no correlation could be determined. However, the results provided valuable information that may suggest that family functioning, quality of life, and perception of illness play an important role in the management of asthma and diabetes in Puerto Rican children. As well as potentially help increase adherence to treatment, that will close care gaps that are greatly impacting Puerto Rican population.

There was a time when abnormal child psychology was the stepchild of abnormal psychology, with perhaps one or two chapters in an entire advanced textbook devoted to children. Given the explosive amount of new research on child development in general since the 1980s, "stepchild" is obviously no longer a valid characterization. Indeed, in the last 15 years, many new journals devoted to childhood problems have made their appearance on library bookshelves. The current study examined parents' reports of change in the parent-child relationship and family functioning following the onset of Pediatric Acute-Onset Neuropsychiatric Syndrome (PANS)/Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections (PANDAS) in their child. Research questions explored 1) how families' day-to-day lives and functioning changed since the onset and 2) what challenges the families faced in raising a child with PANS/PANDAS. Research was conducted in two phases: narrative thematic analyses of online parent forum threads followed by semi-structured interviews with parents. Four primary themes were compiled from analyses of both data sources: Determinants of Family Functioning; Close Relationships; Routines and Daily Life; and Family Emotional Well-Being. It was found that depending on the state of the child (i.e., well or in flare/exacerbation) the family's level of functioning fluctuated. Also impacting family functioning was lack of support from the medical community, child's school system, and from extended family and friends. The marital relationship was found to be greatly impacted, in addition to strains in the parent-child and sibling relationships. Magnified by the uncertainty that surrounds the experience, parents expressed feelings of caregiver burden which often included a transition out of a job to full-time caregiver. Analyses also revealed frequent use of language indicative of trauma to illustrate the experience. The use of trauma metaphors supports the notion that the ability to maintain family functioning is not solely a factor of resilience or preparedness and is unlike that of most childhood chronic illnesses.

Decades of research have demonstrated that the parent-child dyad and the environment of the familyâ€"which includes all primary caregiversâ€"are at the foundation of children's well- being and healthy development. From birth, children are learning and rely on parents and the other caregivers in their lives to protect and care for them. The impact of parents may never be greater than during the earliest years of life, when a child's brain is rapidly developing and when nearly all of her or his experiences are created and shaped by parents and the family environment. Parents help children build and refine their knowledge and skills, charting a trajectory for their health and well-being during childhood and beyond. The experience of parenting also impacts parents themselves. For instance, parenting can enrich and give focus to parents' lives; generate stress or calm; and create any number of emotions, including feelings of happiness, sadness, fulfillment, and anger. Parenting of young children today takes place in the context of significant ongoing developments. These include: a rapidly growing body of science on early childhood, increases in funding for programs and services for families, changing demographics of the U.S. population, and greater diversity of family structure. Additionally, parenting is increasingly being shaped by technology and increased access to information about parenting. Parenting Matters identifies parenting knowledge, attitudes, and practices associated with positive developmental outcomes in children ages 0-8; universal/preventive and targeted strategies used in a variety of settings that have been effective with parents of young children and that support the identified knowledge, attitudes, and practices; and barriers to and facilitators for parents' use of practices that lead to healthy child outcomes as well as their participation in effective programs and services. This report makes recommendations directed at an array of stakeholders, for promoting the wide-scale adoption of effective programs and services for parents and on areas that warrant further research to inform policy and practice.
