" Background: Diabetes related complications are associated with poor medication and treatment adherence and continue to affect different parts of the globe. Diabetes self-management education has been involved in the prevention of diabetes-related complications among type 2 diabetes mellitus (T2DM) patients. Emphasis was on healthy food choices, increase in physical activities, and medication adherence. Purpose: The purpose of the evidence-based practice (EBP) project was to evaluate the effect of education on T2DM inmates 18 years and older on lifestyle changes such as weight loss. Methods: This was an EBP project conducted on 20 participants with T2DM. Project implementation lasted a total of 10 weeks. Interventions were individualized education sessions focusing on various aspects of diabetes control. Pre- and post-test utilized diabetes knowledge test 2 (DKT2) for interventions. Results: The mean percentage score on the pre-test was 63.65% with a SD of 8.35 compared to post-test score of 89.47% and SD of 3.64. There was a 25.82% increase in participants' knowledge post-intervention. The secondary outcome measured weight changes among the participants. Mean of pre-intervention weight was 234.31 pounds, median of 225 pounds, mode of 214 pounds, and a SD of 40.38. Post-intervention weight had a mean of 231.37 pounds, median of 219 pounds, mode of 256 pounds, and SD of 40.24. The difference between pre-and post-intervention weights was 1.25%. Implications for practice: Nurse practitioners play major roles in leading the interprofessional team and in providing health promotion policies and principles to various groups of patients. Successful planning, implementation, and sustaining continuity of change requires multidisciplinary involvement. Abstract: Objective: To systematically review the effect of self-management education on diabetic control in type 2 diabetes Research design and methods: PubMed was searched for English-language articles published between 2010 and 2013. All the studies were original articles selected manually and used randomized control trials generating results of self-management education in people with type 2 diabetes referring to diabetic control. Relevant data were divided and tabulated into factors of population characteristics, interventions and outcomes. Interventions were classified into three sections as collaborative information intervention, lifestyle intervention, and skills teaching intervention based on the patterns of education. Outcomes were categorized into glycemic control, cardiovascular disease (CVD) risk factors and nephropathy risk factors. Results: A total of 24 studies were identified of initial 41 articles for this review. Effects of self-management education on glycemic control were demonstrated to be positive both in short-term (10 months) follow-up, but more positive effects in short-term follow-up. The same effectiveness happens to CVD risk factors, including lipids, weight and blood pressure. On the other hand, with short-term follow-up, teaching skills intervention of self-management education is more effective than collaborative information intervention and lifestyle intervention on reducing glycemic control and CVD risk factors. Also with long-term follow-up, teaching skills intervention of self-management education had more effectiveness than collaborative information intervention and lifestyle intervention on reducing glycemic control. However, few studies including CVD risk factors in the long-term follow-up, so it is difficult to evaluate the effectiveness of on CVD risk factors with long-term follow-up. Conclusions: Evidences supports the positive effectiveness of self-management education with collaborative information intervention, lifestyle intervention and skills teaching intervention among type 2 diabetes patients on diabetic control, in both short-term follow-up and long-term follow-up, but short-term follow-up is more effective than long-term follow-up. Further research is needed to develop self-management interventions to maintain long-term follow-up effects on glycemic control, CVD risk factors and other diabetes complications. Diabetes self-management education: a powerful tool for diabetes control in IndiaDiabetes self-management education (DSME) is a critical element of care for diabetes management and is a powerful tool for reducing glycosylated haemoglobin (HbA1c), risk for complications, hospital admissions and cost of care.1 The purpose of this study was to determine the impact of DSME in improving the outcomes of diabetes care as measured by glycaemic (FPG, PPG & HbA1C), lipid (total cholesterol[TC], triglycerides[TG], low density lipoproteins[LDL] and high density lipoproteins [HDL]) and anthropometric parameters (weight, BMI & waist circumference) in individuals with type 2 diabetes mellitus (T2DM).A retrospective analysis was performed for 160 adult T2DM patients who received DSME training in 2013u20132014 from our center. DSME provided to the study participants was consistent with the requirements from the National Standards for Diabetes Self-Management Education.1 Participants received one time 6 hours intensive group training. All education was provided by a team of diabetologists, certified diabetes educators who in addition are dietician and exercise physiologist. Patients received instruction in: diet/exercise education; self-monitoring of glucose levels; medication management specifically for insulin users; diabetes related problem solving, and lifestyle changes to help reduce the risks and complications of diabetes. Subjects were judged for their acquired knowledge about the disease by using a set of questionnaire and were awarded a score based on their performance. Their socioeconomic status was also ascertained by using Kuppuswammy scoring (KS score)2 which looked at their education, monthly income and occupational status. The subjectsu2019 glycaemic parameters, lipid parameters and anthropometric measurements were checked before the DSME programme and at the end of 6 months and 12 months respectively. The DSME scores were divided into tertiles and lower tertile patients were compared to intermediate and higher tertile patients, to assess, whether differences in acquired knowledge about diabetes had impact on glycaemic, lipid and anthropometric parameters. It was revealed that waist circumference increased in all the three tertiles over the entire year following DSME without any statistically significant change in the weight and BMI over the year. Subjects with DSME score in the lowest tertile had an initial improvement in the FPG values at 6 month but a rebound increase at the end of the year made it lose statistical significance. There was however no statistically significant change in the HbA1C and PPG over the year. The lipid parameters (TC and LDL) showed statistically significant progressive deterioration which perhaps emphasises the need for an intermittent reinforcement of education. Subjects with DSME score in the intermediate tertile had an improvement in the glycaemic parameters (FPG and HbA1C) throughout the year but there was no improvement in the lipid parameters . Subjects with DSME score in the upper most tertile were already having better controls of their glycaemic parameters at the baseline and did not show significant improvements over the year but their lipid parameters (TC and LDL) did show a significant improvements over the year, perhaps secondary to their better understanding of the contribution of lipid control towards prevention of macro vascular complications. Further analysis of the data also revealed that DSME scores were independent of socioeconomic status even when analysed separately for income levels and educational qualification. In India where the large number of diabetic population has a considerable economic implication on the state as well as on the individual, more widespread use of DSME can be a cost-effective option in fighting against the growing menace of the diabetes epidemic. In conclusion, comprehensive diabetes outcomes were correlated with adequacy of baseline diabetes education. Subjects with even minimal diabetes education (lowest tertile) had transient improvement in outcomes whereas those with optimal education had sustained multifactorial benefits. Moreover DSME benefits were independent of socio-economic and educational status indicating its widespread applicability even in resource limited settings. In India where the large number of diabetic population has a considerable economic implication on the state as well as on the individual, more widespread use of DSME can be a cost-effective option in fighting against the growing menace of the diabetes epidemic.References :1. This project is aimed to explore the benefit of diabetes self-management education (DSME) on the self-management of type 2 diabetes mellitus (T2DM). The review of literatures shows the increased concern about the development of type 2 diabetes related complications. The use of diabetes self-management education as revealed by studies helps in a tremendous way in the self-management of type 2 diabetes, thereby helping to reduce the incidence of the complications that occur from poor management of T2DM. The complications when they occur, leads to increased frequency in hospital admission, long hospital stay, high cost of treatment, increased morbidity and mortality rates. Diabetes education has been an ongoing process in the management of diabetes, but the cases of increased hospitalization and development of complications still exist. The aim here is to create more awareness through education for self-management of the condition by the patients with T2DM. The education should be of self-management approach, and not the traditional instructional approach that leaves the patient dependent on the clinicians only for the management of the ill-health. The various literatures reviewed support the use of DSME to achieve this purpose of engaging, empowering, and encouraging the T2DM patients to take control of their diabetes care. The nurses as well as other clinicians can make this happen by reinforcing what is taught and encouraging their patients to adopt the self-management approach. The implication to nursing practice is in the need for nurses to help patients with T2DM to embrace this self-management education approach, and physician/other clinicians should endeavor to refer their patients with T2DM to diabetes classes.

This study compared the effectiveness of diabetes self-management education (DSME) methods by examining changes in knowledge, attitude, and behaviour (KAB) after receiving education. Participants from a convenience sample were randomized into two groups, one receiving education through conversation maps and the other through traditional group education. Participants' knowledge and attitude changes were measured by using a repeated measures pre-test/post-test design and changes in Hb A1c were observed. Focus groups were conducted after education was received to obtain perceptions and self-reported behaviour changes. Significant knowledge and attitude score changes were observed in the conversation map group after education. When comparing the difference in attitude score changes between groups, significant improvements in attitude scores were observed in the conversation map group directly and at three months after education. These changes may lead to improved diabetes self-management, reducing the development of costly health complications related to poorly controlled diabetes. Insight was gained on how DMSE influences changes in KAB.

Type 2 Diabetes Mellitus (T2DM) currently affects approximately 1 in 9 individuals in the United States. Access to education, transportation to appointments, and other social determinants of health can reflect poor self-management, adverse outcomes, and increased morbidity and mortality. The purpose of this quality improvement (QI) project was to evaluate the effectiveness of implementing a series of expert-led, educational classes with a toolkit for patients diagnosed with prediabetes or T2DM. The Health Belief Model was the framework used to develop and implement educational intervention through a 4-month period. This project was conducted at two sites in Northeast Kansas: one which provides primary care services to an underserved population, while the other is a satellite clinic located within a housing authority. The PDSA cycle was used to develop and evaluate program implementation throughout the project. A toolkit was developed and provided to individuals who attended the sessions. Educational sessions focused on T2DM overview, risk factors, comorbidities, medication, diet, exercise, stress, and self-management. Expert specialists were recruited for two of the educational sessions. A diabetes self-management questionnaire (DSMQ) score and glycated hemoglobin (A1c) value were obtained prior to the first educational session. A final DSMQ score and A1c value were obtained at the conclusion of the last educational session. Qualitative measurement revealed improved perceived self-management of T2DM as well as an overall improvement of A1c control after a 4-month period.

As a result, identifying DSM behaviors and diabetes management self-efficacy and examining their effects on glycemic control, as well as identifying which areas of diabetes management self-efficacy and DSM behaviors patients need additional support in, can assist health care teams in achieving better diabetes control.Aim:To determine the prevalence of diabetes self-care activities among patients with type 2 diabetes and to examine the association between socio-demographic and clinical parameters, diabetes self-care activities, and glycemic control among patients with type 2 diabetes.Methods:A cross sectional study was conducted among 123 patients with diabetes at the Sultan Bin Abdulaziz Humanitarian City (SBAHC) in Riyadh, Saudi Arabia. In order to be included in the study, patients must have been diagnosed with type 2 diabetes, they had to be 25 years of age and above, mentally competent, and able to communicate verbally and provide informed consent. Patients with type 1 diabetes, pregnant women, patients experiencing cognitive impairment and any patient who did not agree to participate in the study were excluded. Interviews took place in the visit room individually and lasted approximately 15 minutes. Subjectu2019s verbal and written consent was obtained.The data were collected via medical records and a face-to-face interview was undertaken with consecutive patients attending SBAHC. During the interview, the Diabetes Management Self-Efficacy Scale (DMSES) and the revised Summary of Diabetes Self-Care Activities Scale (SDSCA) questionnaires were completed for each participant. A regression model was used to examine the variables that predicted glycemic control. Moreover, a regression analysis examining the effect of each self-efficacy subscale on its respective diabetes self -management (DSM) behavior was carried out.Result:The most frequently reported DSM behaviors were foot care (mean u00b1 SD = 4.4 u00b11.83, median = 4.5) followed by medication taking self-management behavior (mean u00b1 SD= 4.15 u00b11.68, median = 3.5). Diabetes mellitus significantly affects the health status of adults living in the United States. This is true particularly of African Americans who are disproportionately affected by this devastating chronic condition. As such, the purpose of this study was to analyze the type of education programming and the scope of diabetes self-management education (DSME) received, on the self-efficacy of adults diagnosed with type 2 diabetes. Conducted among adults, particularly African Americans diagnosed with diabetes, this comparative analysis examined the influence of programming on self-efficacy among subjects who participated in educational enrichment provided by faith-based organizations (FBO) and their counterparts who received DSME in clinical care settings only. The Independent Sample T-test, Multiple Regression, and Pearson's Correlation were the statistical tests used to analyze data. Findings revealed a statistically significant difference in diabetes self-management education (DSME), including diet, physical activity, and glucose monitoring between the two groups. Further, age, income, and participation in an FBO program explained 33% of the variance in the final regression model. Finally, data analyses revealed that there was a positive relationship (Pearson's correlation statistic r = 0.26) between diabetes self-management education and self-efficacy. Although analyses suggested participating in FBO programming did not influence self-efficacy, it was confirmed that subjects who participated in such enrichment did receive additional DSME. Based on these findings, it is recommended that medical providers, health educators, and public health professionals collaborate with the faith community to plan, implement and evaluate educational interventions to enhance self-efficacy through DSME.
