In 1993, an International Task Force for Disease Eradication evaluated over 80 potential candidate diseases and made recommendations. However, little has been done to develop the science of eradication systematically. This book reports the findings of a multidisciplinary workshop on the eradication of infectious diseases. It reviews the history of eradication efforts and lessons from previous campaigns and distinguishes among eradication, elimination, and control programs and extinction of an etiologic agent. It addresses a wide range of related issues, including biological and socio-political criteria for eradication, costs and benefits of eradication campaigns, opportunities for strengthening primary health care in the course of eradication efforts, and other aspects of planning and implementing eradication programs. Finally, it stresses the importance of global mechanisms for formulating and implementing such programs.

Since smallpox eradication, the science of eradication has changed and with it, our definitions of what diseases are possible to eradicate. However, eradication must not beget complacency. As has been learned from past control or eradication attempts with a variety of viral diseases, from yellow fever to influenza, accidental or intentional reintroduction is a real threat-one that could strike anywhere and for which we need to be fully prepared. The criteria for assessing eradicability of polio, measles, and other viral infections have been debated extensively. With the elimination and eradication of several viral diseases on the horizon, issues surrounding the cessation of immunization activities become exceedingly important. In an effort to better understand the dynamics of disease eradication and post-immunization policies, the Institute of Medicine Forum on Emerging Infections hosted a two-day workshop (February 1-2, 2001) on The Consequences of Viral Disease Eradication. This book explores the principles underlying the biological challenges, medical interventions, the continuing research agenda, and operational considerations for post-immunization strategies for vaccine-preventable viral diseases, and highlights important efforts that may facilitate wise decision making.

Eradication of infectious disease is a public-health concept of the current generation. It involves world-wide elimination of the specific pathogen of each infectious disease from its susceptible host since regional eradication may be transient with reintroduction of the infection by foreign carriers or vectors. Even with world eradication of infection, a similar infection may evolve from related organisms or mutations unless the potential pathogen is destroyed in situ. Then, too, non-pathogenic organisms replacing eradicated pathogens in the human body may become a greater cause for alarm. The future of infectious diseases will depend on the ingenuity and resources of man coping with these problems. Man must live with his infection in a state of ecological equilibrium which cannot be disturbed by eradication even in the face of possible invasion by extraterrestrial microorganisms. A germ-free world is an ecological absurdity just as a perpetual motion machine is a mechanical absurdity. Yet, it seems hopeless to achieve an ecological equilibrium between microbes and man when history records such disastrous epidemics comparable to the destructive effects of total warfare. Provocative causes of microbial disease upsetting the ecological equilibrium invoke marked disturbances in man's external and internal environments from which pathogens can never be eradicated. Microbial disease is an inevitable consequence of life in an unstable world. The multiplicity of determinants which affect ecological equilibria limits the power of the experimental method to predict trends. Nevertheless, it has already been possible to produce a world environment in which most people can go through life without any serious infectious disease except the uncontrollable common cold. Infectious diseases should be rare indeed by the end of the century. - Foreword.

Introducing a book on global infectious diseases is a daunting challenge. Modern developments in molecular research, newly available opportuni ties for earth-orbiting, satellite-based surveillance as a means of predicting certain regional epidemics, and the introduction of remarkable new anti biotics which can cure some awesome problems such as river-blindness would each have astonished even the most jaded reader only a few decades ago. Just the same, even with these laudable advances, malaria is still with us and both poliomyelitis and tuberculosis are growing worldwide problems. As a holdover from my own medical school days, I must confess to an affection for the older term of "tropical medicine" and its historic context, in preference to the more popular present name which is the title of this book. A century or two ago the motives to study tropical medicine were obvious: nations having enormous amounts of maritime trade with distant lands established colonial empires where health in the colonies was a con cern for both the natives and the colonists. It is unsurprising that the great institutes for such studies sprang up in London, Amsterdam, Hamburg, New York and New Orleans, but few of these centers and other similar ones have continued to flourish or even to survive, perhaps because the days of empires have gone.
