The German health care system is on a collision course with budget realities. Costs are high and rising, and quality problems are becoming ever more apparent. Decades of reforms have produced little change to these troubling trends. Why has Germany failed to solve these cost and quality problems? The reason is that Germany has not set value for patients as the overarching goal, defined as the patient health outcomes achieved per euro expended. This book lays out an action agenda to move Germany to a high value system: care must be reorganized around patients and their medical conditions, providers must compete around the outcomes they achieve, health plans must take an active role in improving subscriber health, and payment must shift to models that reward excellent providers. Also, private insurance must be integrated in the risk-pooling system. These steps are practical and achievable, as numerous examples in the book demonstrate. Moving to a value-based health care system is the only way for Germany to continue to ensure access to excellent health care for everyone.

Nevertheless, medical facilities increasingly recognise that their resources are limited and that they are subject to mechanisms of other markets, meaning that they need to ensure the own position in the health care market more than ever (Schreyögg, 2000, p. 185). Increased competition between hospitals, scarce resources, and amendments of legislation are forcing every provider of medical services to consider a sustained and efficient structure of the provision of their services over and above a strong customer orientation. With several reforms coming from the government, which were supposed to retain an eminent level of public health care and at the same time putting a halt on the consistently rising health care costs, the hospital was now heavily reliant on the number of patients treated. Thus, hospitals were faced with the challenge of retaining or gaining more patients, while concurrently having an unchanged or lower budget, which meant that the role of the hospital changed from being a mere renderer of medical services to having the function of an economic actor. Concretely, this implied for hospitals adopting a strategic concept that is aligned to the market and to the heterogeneous target groups of this market. The health care sector in Germany has become increasingly important in the scientific field of information systems over the last decades. This development has been reinforced by the demographic change and, more recently, by the current Covid-19 pandemic. Irrespective of the field of work and application, digital interface processes, data acquisition, data exchange, as well as data preparation, analysis, and correct archiving are undoubtedly gaining in importance. They also enable new possibilities for service design and the corresponding provision of both digitized supply services and digitized information services. Against the background of increasing and more complex requirements in the health care system, digitalization can become a very important factor in the provision of services. This is especially true for hospitals. In hospitals, increasing data volume and level of detail increases the complexity of data documentation. Hospital staff currently spend a lot of time on data acquisition and transfer, and correspondingly less time is available for the beneficial core work with patients. Therefore, digitization and automation strategies through patient-oriented interactions are becoming increasingly necessary. Patient portals are one possible way to enable care and new services and to reduce the effort for comprehensive data collection. This paper presents the current situation in Germany regarding patient-oriented solutions by capturing, structuring, and analyzing the market of existing patient portals in German hospitals. Existing portal functionalities are identified and patient portals are evaluated against these functionalities. Our results show that there are only two patient portal solutions in the largest hospitals in Germany and that the existing portal solutions by far do not completely fulfill all functional categories. Based on these results, we develop and discuss further research questions for future research. This includes especially design-oriented research with regards to the digital patient journey.
