Excerpt from The Mechanism and Graphic Registration of the Heart Beat It is now some eight years since I published, under the tittle of "The Mechanism of the Heart Beat," a book in which I attempted to review a collection of numerous, and at that time recent, researches upon normal and disordered action of the heart beat. The subject-matter at my disposal had been gathered by precise methods of research; it illustrated the application of such methods to clinical studies; it emphasised the value of reproducing in animal experiment changes witnessed during the progress of human maladies. For the last-named reason a number of clinical and experimental observations were placed side by side in the book so that they might be compared closely with each other. My general purpose was to show the solid progress which may be achieved by exacting methods and to preach an assiduous gathering together of all evidence in support of any given proposition. For it was in my mind, and still is, that clinical pathology, by which I mean the study of disease in the living man, suffers both from inexact observation and from hasty conclusion. Many methods of clinical observation, being subjective, lack accuracy. Some subjective methods are commonly believed to be effective only in the hands of a chosen few; such methods, while always open to scepticism, never can be productive. In no science, in the true sense of the word, is there the almost open vaunting of and reliance upon personal skill, in no science is there the veneration of supposed manipulative dexterity which we discover in our realm of medicine. In the denervated state the mammalian heart, both in vivo and in vitro, is excited at very regular intervals, the coefficient of variance of the interbeat intervals not exceeding 2%. The pacemaker that is the source of this regular ex citation is localised normally within the sinus node (" sino-atrial node " node of Keith and Flack), a most intriguing small piece of tissue in the caval corner of the right atrium. A small portion of this node containing a group of probably only a few thousands of cells fires spontaneously, that means without any exter nal influence to trigger their activity. The so called pacemaker cells do this by letting their membrane potential fall to the level where an action potential will start which subsequently activates surrounding cells to fire an action po tential. The first question which is tackled in this book is which processes underly this spontaneous diastolic depolarization. This is discussed in section I, concerning the fundamental properties of pacemaker cells with special refer ence to ionic membrane currents. Although views still quite differ about the exact nature of the membrane processes that cause the automatic pacemaker dis charge there is agreement that diastolic depolarization is brought about by the interaction of a number of ionic current systems, including both inward and out ward going currents.
