Opioids have played a prominent role in society for centuries and have been lauded for both their analgesic and euphoric qualities by cultures throughout the world. The advent of medical and pharmaceutical sciences in the 20th century ushered in a wide variety of different commercial opioid products that were designed to maximize their therapeutic potential. As the use of opioids increased, a corresponding need emerged to understand more about how opioids exert their effects in the body and brain; the consequences of opioid use regarding physical dependence, withdrawal, and craving; how we might best treat opioid use disorder (OUD) and address risk for opioid-related overdose; and how opioids may intersect with other clinical conditions to produce unique challenges. Edited by Kelly E. Dunn, The Oxford Handbook of Opioids and Opioid Use Disorder synthesizes research across the spectrum, and establishes a foundational knowledge regarding historical and current epidemiological trends, neurobiological and genetic contributors to opioid effects and OUD, and core elements of opioid use such as withdrawal and craving. It provides specific information and guidance regarding opioid treatment paradigms, including chapters on specific pharmacotherapies as well as treatment approaches, and discusses considerations for special clinical populations. An overview of these respective issues is provided across 38 chapters, which outline the history and science of these topics alongside clinical considerations and case examples. Written by expert bench scientists, epidemiologists, clinical trial interventionists, medical practitioners, and harm reduction specialists, this handbook will serve as a comprehensive guide for practitioners, policymakers, students, and researchers who wish to achieve a better understanding of the complex world of opioid and OUD practice and science.

Opioids are responsible for the vast majority of preventable deaths related to drug use, and opioid crisis is a serious international health problem. Opioids are also the mainstay in pain treatment and are invaluable tools in palliative care. The purpose of this book is to present an overview of the complex field of opioid pharmacology through evidence-based chapters in clear language backed up with sound scientific evidence, providing researchers and health professionals with a firm understanding of their effects and mechanisms of action. The authors present an overview of the history of opioids from ancient civilizations to the current opioid crisis, covering state-of-the-art advances on opioid-induced signal transduction, opioids in pain management, and the neuroinflammatory effects of opioids. Also discussed are opioid use disorders and their treatment. The authors also review the growing body of evidence of opioid effects on innate and adaptive immune responses, discussing the molecules involved in the crosstalk between opioids and innate immunity receptors to provide an updated view of those compounds as important regulators of inflammation and host defense against pathogens and damage. The overall purpose is to provide the reader with a deeper knowledge of this field despite the complexities associated with the existence of numerous ligands, receptor subtypes, and complex pharmacological profiles. To this end, the chapter authors are specialists in the field who also have extensive experience in teaching, hoping to lend their expertise in translating complex concepts into comprehensive and clear explanations.

Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Substance Misuse and Young People: Critical Issues is a comprehensive source of information on young people’s requirements for assessment, treatment and other interventions because of their misuse of substances. It highlights approaches that enhance understanding of the routes that lead young people to substance misuse and also the routes away from it. The emergence of new substances and methods of misuse makes this ever more relevant. The authors are international experts in the fields of psychiatry, paediatrics, medicine, psychology, genetics, resilience, neuropharmacology and epidemiology. This book acknowledges how widespread both substance misuse and psychiatric disorders are and explores the complex, challenging links between co-occurring conditions. Use of substances is associated with illness and premature mortality, and more so for people who have combined disorders. The authors critically assess the vital need for intervention during adolescence and early adulthood. They provide detailed clinical views of the psychosocial interventions and medications currently available and illustrate them with case studies that emphasise adolescents’ experiences and thoughtful lifestyle-specific interventions. This book provides theoretical knowledge and indicates the practical skills that practitioners require for work with young people who misuse substances. It is highly applicable to medical practitioners, psychologists, pharmacists, social workers, police officers, probation officers, educationalists and related social and healthcare professionals.
