In the wake of structural adjustment programs in the 1980s and health reforms in the 1990s, the majority of sub-Saharan African governments spend less than ten dollars per capita on health annually, and many Africans have limited access to basic medical care. Using a community-level approach, anthropologist Ellen E. Foley analyzes the implementation of global health policies and how they become intertwined with existing social and political inequalities in Senegal. Your Pocket Is What Cures You examines qualitative shifts in health and healing spurred by these reforms, and analyzes the dilemmas they create for health professionals and patients alike. It also explores how cultural frameworks, particularly those stemming from Islam and Wolof ethnomedicine, are central to understanding how people manage vulnerability to ill health. While offering a critique of neoliberal health policies, Your Pocket Is What Cures You remains grounded in ethnography to highlight the struggles of men and women who are precariously balanced on twin precipices of crumbling health systems and economic decline. Their stories demonstrate what happens when market-based health reforms collide with material, political, and social realities in African societies.

The global rise in pandemics, most recently COVID-19, and other health challenges, some of which are due to climate change, have imposed significant challenges on the healthcare systems in economies around the world. Thus, this book deals with an issue that is very timely and relevant, not just in Africa but globally. It critically assesses healthcare reforms in Ghana under the Fourth Republic, since 1993. Although it focuses on Ghana’s National Health Insurance Scheme of 2003, the book instructively goes beyond this program. The book argues that, although Ghana is a bellwether of healthcare reforms in Africa, its healthcare initiatives are still far from the service haven of healthcare as a human right. Themes that animate the book’s argument include the need to translate human rights law, such as the right to health, into practical policies that work for ordinary citizens. Key highlights of the book include an increased accent on health as a human right, emphasis on comparative analysis in healthcare studies, and the formulation of a four-hallmark framework, embedded in economics, law, politics, and human rights, to act as a guide for assessment of healthcare reforms in Africa in particular, and Ghana more specifically. Using Ghana as a case study and analytical window into the world, the book offers a valuable and timely resource for academics, students and policymakers across the disciplines of development and healthcare economics, law, public policy, political science, sociology, and African and Caribbean studies, as well as in various fields in health science.

The issue of universal and free access to treatment is now a fundamental goal of the international community. Based on original data and field studies from Brazil, Thailand, India and Sub-Saharan Africa under the aegis of ANRS (the French nationalagency for research on Aids and viral hepatitis, this timely and significant book both assesses the progress made in achieving this objective and presents a rigorous diagnosis of the obstacles that remain. Placing particular emphasis on the constraints imposed by TRIPS as well as the poor state of most public health systems in Southern countries, the contributing authors provide a comprehensive analysis of the huge barriers that have yet to be overcome in order to attain free access to care and offer innovative suggestions of how they might be confronted. In doing this, the book renews our understanding of the political economy of HIV/AIDS in these vast regions, where the disease continues to spread with devastating social and economic consequences. This volume will be a valuable addition to the current literature on HIV/AIDS in developing countries and will find widespread appeal amongst students and academics studying economics, sociology and public health. It will also be of interest to international organizations and professional associations involved in the fight against pandemics.

This handbook provides a comprehensive and critical overview of the gamut of contemporary issues around health and healthcare from a political economy perspective. Its contributions present a unique challenge to prevailing economic accounts of health and healthcare, which narrowly focus on individual behaviour and market processes. Instead, the capacity of the human body to reach its full potential and the ability of society to prevent disease and cure illness are demonstrated to be shaped by a broader array of political economic processes. The material conditions in which societies produce, distribute, exchange, consume, and reproduce – and the operation of power relations therein – influence all elements of human health: from food consumption and workplace safety, to inequality, healthcare and housing, and even the biophysical conditions in which humans live. This volume explores these concerns across five sections. First, it introduces and critically engages with a variety of established and cutting-edge theoretical perspectives in political economy to conceptualise health and healthcare – from neoclassical and behavioural economics, to Marxist and feminist approaches. The next two sections extend these insights to evaluate the neoliberalisation of health and healthcare over the past 40 years, highlighting their individualisation and commodification by the capitalist state and powerful corporations. The fourth section examines the diverse manifestation of these dynamics across a range of geographical contexts. The volume concludes with a section devoted to outlining more progressive health and healthcare arrangements, which transcend the limitations of both neoliberalism and capitalism. This volume will be an indispensable reference work for students and scholars of political economy, health policy and politics, health economics, health geography, the sociology of health, and other health-related disciplines.

Recent political, social, and economic changes in Africa have provoked radical shifts in the landscape of health and healthcare. Medicine, Mobility, and Power in Global Africa captures the multiple dynamics of a globalized world and its impact on medicine, health, and the delivery of healthcare in Africa—and beyond. Essays by an international group of contributors take on intractable problems such as HIV/AIDS, malaria, and insufficient access to healthcare, drugs, resources, hospitals, and technologies. The movements of people and resources described here expose the growing challenges of poverty and public health, but they also show how new opportunities have been created for transforming healthcare and promoting care and healing.

Health care is absorbing an increasing share of resources in all countries. It is the task of public policy to ensure that the nation secures good quality attention at an affordable price. This book, distinguishing clearly between health status and health care, examines the ways in which governments can keep down morbidity and mortality while also ensuring that treatments are medically justifiable, economically cost-effective and socially equitable. Ignorance, uncertainty, asymmetrical information, professional monopolies, insurance and poverty all mean that the market by itself is not enough. Pragmatic intervention is also required to ensure that well-being and social justice are delivered in combination with value for money. Health Care and Public Policy is a comprehensive and intelligible cross-disciplinary account of the objectives of health care policy (medical, social, economic) and of the policy-tools that government can employ (cost benefit analysis, entry barriers, competition) in order to ensure that scarce resources are not wasted nor needy social groups deprived of basic and affordable care. Health policy is hotly debated in all countries. This cross-national book is timely and relevant. It will be of special interest to academics and students who want to deepen their understanding of health economics, social policy and administration, public policy and government, political economy and the logic of institutions.
