Postpartum is a vulnerable period that calls for careful monitoring, support, and anticipatory guidance. More than half of pregnancy-related deaths occur postpartum, with substantial morbidity occurring in the early postpartum period. Depression during or immediately after pregnancy is a common medical complication affecting one in seven women in the United States. There is a large body of research that supports earlier and more frequent postpartum care for all women. Healthcare governing bodies, including the American College of Nurse Midwives (ACNM) and the American College of Obstetricians and Gynecologists (ACOG) currently have guidelines in place for more frequent postpartum visits. There is a gap between research evidence and current practice related to postpartum visits; therefore, the project was translating evidence into practice. The project design was comparative, pre-and post- (retrospective and prospective). The project’s primary aim was to compare postpartum depression rates in women who received postpartum care with a six-week postpartum visit only (the pre-group) with women who received optimized postpartum care with the addition of a one-week visit in the office and a three-week virtual or in office visit if needed (the post-group).Mothers in the pre-practice change groups had one six-week postpartum visit. Mothers in the post-practice change group were scheduled for a one-week appointment in the office after delivery and a six-week comprehensive postpartum visit. Mothers with an Edinburgh Postnatal Depression Scale (EPDS) score of eight or greater, a history of a postnatal mood disorder, or a history of a mood or psychiatric disorder all returned for an additional three-week postpartum visit. The project included 297 women, 185 in the pre-practice change group and 112 in the post-practice change group. The results revealed a decrease in the mean EPDS score at six weeks postpartum in the post-practice change group (M = 6.2), compared to the mean EPDS score in the pre-group (M = 7.8). Early postpartum visits had a statistically significant impact overall on postpartum depression symptoms (p = 0.28). The addition of early postpartum visits positively influenced the presence and severity of postpartum depression symptoms, and therefore could decrease maternal and infant morbidity and mortality.

Background: One out of eight women experience depressive symptoms during the postpartum period. In addition, approximately 63.8% of new mothers are affected by fatigue during the postpartum period, making this the most common problems affecting women during that period. Aims: The purpose of this study was to explore the relationships among postpartum fatigue, depressive cognitions, resourcefulness, quality of life and life satisfaction of the Arab postpartum mother. Additionally, the study examined if there was a mediating effect of resourcefulness on the relationship between the process regulators (DC and PPF) and the mother's quality of life outcomes (quality of life and life satisfaction) during the postpartum period. Theoretical Framework: The conceptual framework for this study based on the middle range theory of resourcefulness, which was developed by Zauszniewski in 2006. Methods: A cross-sectional descriptive design was used with 123 postpartum women who gave birth less than 6 months ago. "What's App" and Facebook were used for recruitment. The survey was constructed in both Arabic and English. The self-administered online survey was developed in Qualtrics and data were collected from January 6th to February 6th, 2017. Linear regression was used to address the research questions and to test the mediating effect of resourcefulness. The current study investigated potential links between maternal postpartum depression (PPD), quality of mother-child relationships, and children's early language development. The study involved a secondary data analysis of a postpartum depression (PPD) treatment efficacy study involving 147 randomized mother-child dyads. Mothers in the study met criteria for major depressive disorder in the postpartum period, and children were between 2 weeks and 7 months of age at the time of intake. Longitudinal data were collected for measures of PPD severity, PPD chronicity, quality of the mother-child relationship, and early language development. The current study had three aims: (a) to investigate a possible direct effect of PPD dimensions (severity and chronicity) on children's early language development; (b) to investigate the quality of the mother-child relationship as a possible mediator of the effects of PPD severity and chronicity on children's early language development; and (c) to investigate sociodemographic variables (maternal education and child gender) as possible moderators of these effects. Statistical analyses included tests of indirect effects and tests of conditional indirect effects. Results supported the existing research, indicating that a statistically significant suppression effect for PPD chronicity on early language development was partially accounted for by the quality of the mother-child relationship, especially in mother-son dyads. The negative direct effect of PPD chronicity on early language development was strongest at the lowest level of maternal education. Findings hold implications for future research, policy, and practice.

"Postpartum sleep disturbance is highly prevalent, especially in first time mothers. More importantly, sleep disturbance is associated with postpartum depression, and is independent of other risk factors. Sleep disturbance may be a modifiable risk factor for postpartum depression but few studies have examined interventions to improve postpartum sleep. The purposes of this pilot randomized controlled trial were to determine: 1) the feasibility, facilitators, and barriers of the Sleep Support for Moms Intervention (SSMI), and the smart phone sleep application, Sleep Time (“Azumio Inc., 2013”), as an objective measure of sleep in first time postpartum mothers, and 2) the effect of the SSMI on sleep disturbance and depressive symptoms in the mothers and on the sleep of their infants. The conceptual framework that guided this study was the Health Belief Model, which best supports how the mothers are influenced to be proactive in their health. First time mothers (n = 40) from a suburban Mid-Atlantic nonprofit, obstetric clinic, serving low income women, were consented and randomized to the SSMI or the active control group. The researcher provided the SSMI sessions consisting of a 45 minute, prenatal anticipatory guidance class on improving sleep and the risk for postpartum depression. The SSMI included: awareness of the risk for postpartum depression due to sleep disturbance, enlisting family/partner support, room sharing, enhancing infant sleep, sunlight exposure, sleep hygiene, and weekly texting. The active control group received a class identical to the SSMI group, except the topic was infant care and safety. Sleep was measured subjectively with reports of refreshed sleep, nighttime sleep, and the General Sleep Disturbance Scale, and objectively with the sleep application. Depression was measured by the Edinburgh Postnatal Depression Scale. The Positive and Negative Affect Schedule was used in additional exploratory analyses. The women reported nighttime sleep totals daily by cellular phone texting for the 6 weeks of postpartum. The final sample was 34 women (mean age 26.6 years) from varying racial/ethnic backgrounds. The SSMI was found to be acceptable as evidenced by minimal attrition (15%), and an adherence of 92%. The use of the sleep application was not feasible. The SSMI group had significantly better subjective sleep and fewer depressive symptoms when compared to the active control group. Infant sleep was not significant between the groups. In additional analyses the SSMI group had less negative affect compared to the control group. These findings suggest that anticipatory guidance on simple sleep strategies provided by a health professional can improve sleep and decrease depressive symptoms in first time mothers."--Pages [x-xi].
