In 1951 WOLFGANG MAUERMAYER was one of the first young German urologists to visit the United States, after the war. He brought a very personal enthusiasm, and joy in learning, to many well known clinics in both the eastern and western United States. It was then that I first had the pleasure of meeting him; and, since then, we have enjoyed each other's company on a number of occasions. From California (and during a half year sabbatical in Berlin) I have followed the course of his exceptional career. He has transformed his Munich Clinic into one of the leading European centers for transurethral surgery. His first book on transurethral operations, published in 1962, appeared only in German. Consequently, it is little known in the United States and England. The present book, now in its first English edition, is the result of more than 30 years experience in transurethral surgery. During this time more than 10,000 patients were treated in Mauermayer's clinic by various endoscopic oper ations. His unusual reservoir of experience forms the basis for this book. It seems of particular importance that surgical techniques are described in a number of steps, since this enables even a novice to understand the various procedures.

In 1963 Professor Huggins! wrote "more than one half of the male population over the age of 50 suffer from benign tumors known as prostatic hypertrophy. Often an enlarged prostate is the only difficulty to cloud an otherwise tranquil old age". This statement emphasizes two important features of benign prostatic hypertrophy (BPH); the frequency with which it occurs and its association with increasing age. The true incidence is difficult to determine as most data relate to selected groups of patients; moreover, the diagnostic criteria of prostatic hypertrophy are not clearly defined. Normality merges imperceptibly into abnormality, for even in men who are clinically normal, rates of urinary flow decrease with 2 increasing age . Despite these inaccuracies, incidence figures show the magnitude of the problem; Flocks (1963) 3 reported that 65% of American men over 60 years suffer from it and in a selected group of Danish men 43% had symptoms of the 4 disease ; similar figures are reported for the U ni ted Kingdom 5. At present most patients with BPH, who need treatment, undergo surgical prostatectomy which may be considered the usual treatment. The frequency of the disease alone, creates certain problems, for annually more than 30000 men are admitted to hospital in the United States with this as a primary diagnosis and with an ageing population the figures can be expected to increase.

In brief, in an experimental animal the ureter of one kidney was ligated and the subsequent renal hypertrophy of its contrala teral mate was studied quantitatively from anatomic and functional stand points. There were two central questions in the Hinman study: How does a normal kidney of an experimental animal recognize that its load has been doubled abruptly? What is the signal for renal hypertrophy? Benign hypertrophy of the prostate is quite different from compensatory hypertrophy of the kidney. It is now known that benign prostatic hypertrophy (BPH) is not a hypertrophy but a benign tumor consisting of a collection of spheroids of micro- and macrodimensions. In technical terms BPH is an adenofibromyoma. Perusal of the present volume will reveal many fascinating facets of BPH of particular interest to urologists and others with an investigative bent of BPH does not occur in children. BPH occurs as a medical rarity in mind.
