Excerpt from The Rheumatic Diseases The term Rheumatism has been gene rally applied to a variety of conditions associated with pain in the joints, muscles, nerves, and fibrous tissues of the body. AS our knowledge of these conditions has progressed there has been an increasing tendency to limit the use of the word in this way, and to employ the words rheu matism and rheumatic only for such conditions as are excited by a specific micro organism, the diplococcus rheumaticus. Used in this way the term rheumatic diseases would include the various manifestations of acute, chronic, and muscular rheumatism in adults and children, and certain cases of acute and chronic rheumatoid arthritis, the exciting cause of which is in our opinion a modified or avirulent form of the coccus of acute rheumatism. Excerpt from A Treatise on Rheumatism and Rheumatoid Arthritis Although the literature of Rheumatism is very extensive, and although the great advances which have been made in our knowledge of the disease of recent years have greatly modified the views formerly held concerning it, yet the various contributions to the subject are, for the most part, scattered through the Transactions of Societies and the Medical Journals, and have not often been brought together in the Treatise form. I therefore felt, in accepting the invitation of my publishers to write a work upon the subject, that I might be doing a service to medical science if I endeavoured to present a consistent picture of Rheumatism as a systemic disease, of which the articular lesions are, at the same time, the most frequent and conspicuous manifestations, but which also implicates many other structures and tissues of the body, and assumes widely different forms in different cases. How far the outcome of this attempt falls short of its aim, I am only too conscious; but I trust that the work will nevertheless prove useful to those who are anxious to form a notion of the present position of our knowledge of the subject. It has been my endeavour to restrict the denotation of the words "Rheumatism" and "Rheumatic," which are often so loosely employed that they have almost forfeited all claim to be regarded as scientific terms, to one definite set of phenomena, which I believe to depend upon one single and specific morbid process, of the nature of which it is not yet possible to speak with any degree of certainty. The latter portion of the book treats of Rheumatoid Arthritis, which I believe to be, unlike Rheumatism, an essentially articular affection, the lesions of which are dependent upon trophic disturbances. Excerpt from Rheumatism: Its Nature, Its Pathology, and Its Successful Treatment "A Perusal of the literature which bears on the question of the treatment of acute rheumatism (rheumatic fever) is a task from which few would rise with any definite idea as to how that disease is best treated. Purgatives, diaphoretics, sedatives, alkalies and alkaline salts, colchicum, aconite, quinine, guaiacum, lemon juice, sulphur, mercury, veratria, tincture of muriate of iron, etc., would each be found to have in turn attracted the favorable notice of one or more of those who have directed attention to the subject. Of all these different remedies not one stands out prominently, as that to which we can with confidence look for good results. We have, indeed, no remedy for acute rheunatism - a. malady which not unfrequently proves fatal, which is always accompanied by great pain, and is a fruitful source of heart disease. "Under these circumstances I need make no apology for bringing under the notice of the profession a remedy which, so far as my observations have gone, has given better results than any which I have hitherto tried - and I have tried all the usual remedies over and over again. "In the course of an investigation into the causation and pathology of acute febrile ailments, which has for some time engaged my attention, I was led to give some consideration to intermittent and to rheumatic fever. The more I studied these ailments, the more was I struck with the points of analogy which existed between them.
