In 2008, Paul Farmer and Jim Yong Kim described global surgery as the "neglected stepchild" of healthcare, and now leaders from around the world are working to redefine it as a human right through the Lancet Commission on Global Surgery. In order to help advance global surgery and anesthesia as a public health intervention, providers require a practical, hands-on manual to train and educate them to provide safe and effective surgery and anesthesia in resource-limited settings. In an effort to respond to that need, over 140 clinicians from high-, low-, and middle-income countries collaborated to create the Global Surgery and Anesthesia Manual: Providing Care in Resource-Limited Settings. This guide provides in-depth instruction on epidemiology and ethics surrounding surgical services, and on anesthesia, perioperative care, trauma surgery, and non-trauma surgery. With contributions from a group of culturally and academically diverse clinicians, each chapter is enriched by at least one commentary from a surgeon or anesthesia provider in a low- or middle-income country (LMIC) who shares first-hand perspectives on the realities and complexities of providing surgical care in these settings with limited resources. Many patients who present to district (first-referral) level hospitals require surgical treatment for trauma, obstetric, abdominal or orthopedic emergencies. Often surgery cannot be safely postponed to allow their transfer to a secondary or tertiary-level hospital but many district hospitals in developing countries have no specialist surgical teams and are staffed by medical, nursing, and paramedical personnel who perform a wide range of surgical procedures often with inadequate training. The quality of surgical and acute care is often further constrained by poor facilities, inadequate low-technology apparatus and limited supplies of drugs, materials, and other essentials. The mission of the team responsible for Clinical Procedures in the World Health Organization Department of Essential Health Technologies (EHT) is to promote the quality of clinical care through the identification, promotion and standardization of appropriate procedures, equipment and materials, particularly at district hospital level. WHO/BCT has identified education and training as a particular priority, especially for non-specialist practitioners who practice surgery and anesthesia. It has therefore developed Surgical Care at the District Hospital as a practical resource for individual practitioners and for use in undergraduate and postgraduate programs in-service training and continuing medical education programs. A critical problem in resource-scarce countries across the globe is the shortage of appropriately trained health care providers. According to the World Health Organization, the current global health workforce shortage of 7.2 million providers is estimated to increase to 12.9 million by 2035. This disproportionately affects resource-scarce countries, denying basic health care to millions and limiting access to life-saving treatments. Due to limited resources in these countries, not enough health professionals receive training, few have the opportunity for continuing education, and the ability to develop or implement educational programs and curricula is constrained. Additionally, many existing providers choose to emigrate in pursuit of professional advancement opportunities, contributing to the overall shortage of qualified health care providers in these environments. Efforts to strengthen health workforce capacity not only increases access, safety and availability of care, but is critical to building resilient health systems capable of caring for the world’s neediest populations. This requires not only cultivating new health care providers, but also providing ongoing professional development to retain and support current providers, advancing the level of practice in accordance with current clinical science, cultivating educators, and enhancing training curricula. It is critical also to contribute to the limited body of research documenting the effectiveness and impact of various models of collaborative education and partnership to improve health worker training and retention. This Research Topic examines strategies for building health workforce capacity through the prism of educational partnerships, offering significant examples of effective models of international collaborative education as well as insight and guidance on the structure and operation of successful global partnerships.
