There is a myth that people with mental disorders comply poorly with treatment. In fact, psychiatric patients are no more likely than patients in other medical specialities to go against the advice of their doctor. That said, it is easy to find instances where psychotropic medication is refused by the supposed beneficiary. The value of neuroleptic treatment in schizophrenia is now widely accepted. Failure to take such treatment is associated with relapse and relapse may endanger the patient and other people. Despite this, people with schizophrenia frequently fail to take their treatment. This study shows that one third can be expected to be non-compliant within two years of leaving a general adult psychiatry ward. It also looks at the reasons for this: the influence of drug side-effects is examined, as well as the impact of each patient's attitude to treatment and whether or not they have stopped taking prescribed medication in the past.

An international team of leading researchers and clinicians here provide a comprehensive, epidemiological overview of this multi-faceted and still perplexing disorder, and address some of the key questions it raises. How important in the genetic contribution to schizophrenia? Do pregnancy and birth complications increase the risk for schizophrenia? Is the incidence of schizophrenia changing? Why is the rate higher among immigrants and in those born in cities? Controversial issues such as the validity of discrete or dimensional classifications of schizophrenia and the continuum between psychosis and 'normality' are explored in depth, and separate chapters are devoted to topics of particular relevance to schizophrenia such as suicide, violence and substance abuse. Finally, new prospects for treatment and prevention are considered. Drawing together the findings from social, genetic, developmental and classical epidemiology of schizophrenia, this text will prove an invaluable resource for clinicians and researchers.

Poor adherence to therapy is one of the main obstacles to treatment effectiveness in schizophrenia. It is the main determinant of relapse, hospitalization, symptom persistence, and poor psychosocial functioning and outcome. Adherence to treatment is affected by various factors related to the disease characteristics, to the patient him- or herself, to the treatment, and to the therapeutic relationship. Some of these factors are modifiable, and both pharmacological and non-pharmacological strategies have been developed for this purpose. This book addresses the different aspects of adherence to treatment in schizophrenia and related disorders in a systematic but easy-to-use manual format. Chapters focus on a full range of issues, including pharmacological and non-pharmacological strategies to enhance adherence and continuity of care, relevant psychological factors, the importance of the patient-doctor relationship, and the need for an alliance with other care-givers. Adherence to Antipsychotics in Schizophrenia will be an invaluable asset for all who are involved in the care of patients with schizophrenia.

Schizophrenia is a serious mental disorder that can devastate the lives of patients and their families. The condition is chronic and may persist throughout patients' lives, disrupting education and work and greatly reducing the quality of life. Affecting nearly 0.8% of the population worldwide, schizophrenia is a serious public health problem demanding improved treatment. Recent progress in neuroscience and biological studies of schizophrenia is promising. However, to understand the concomitant behavioral characteristics and implement effective treatment programs, multifaceted approaches are required. "Comprehensive Treatment of Schizophrenia" was the subject of the 8th Keio University International Symposium for Life Sciences and Medicine, held in Tokyo. From the symposium, 40 papers were chosen to be included here in nine sections covering the broad spectrum of recent clinical progress in the treatment of schizophrenia and allied disorders.

The introduction of antipsychotic agents in the 1950’s substantially improved the treatment of schizophrenia and other psychotic disorders. However, clinical and functional outcomes are still far less than optimal for patients, and have not improved in recent years despite the development of several new antipsychotics. Efficacy rates are further compromised by medication non-adherence, which has been reported to affect more than half of patients. In response to these issues, several non-pharmacological interventions have been developed for the treatment of schizophrenia, such as cognitive behavioral therapy, cognitive remediation, social cognition training and metacognitive approaches. Although these interventions have produced promising results, there is still much controversy regarding their usefulness and applicability in clinical practice. A major impeding factor for their dissemination is possibly a lack of sufficient evidence regarding their specific indications, mechanisms of action, adverse effects, but also practical issues concerning the interpretability of respective clinical studies, such as the choice of outcome variables and control of confounding factors. The present Research Topic includes original research articles and reviews addressing these issues.

Schizophrenia is the archetypal form of madness. Schizophrenia is a common disorder and has a devastating effect on sufferers and their families-patients typically hear voices in their heads and hold bizarre beliefs. The schizophrenic patient presented to the public in sensational press reports and lurid films bears little resemblance to reality of the illness. This book describes what schizophrenia is really like, how the illness progresses, and the treatments that have been applied. It also summarizes the most up-to-date knowledge available about the biological bases of this disorder. Finally it attempts to give some idea of what it is like to have schizophrenia and what this disorder tells us about the relationship between mind and brain. ABOUT THE SERIES: The Very Short Introductions series from Oxford University Press contains hundreds of titles in almost every subject area. These pocket-sized books are the perfect way to get ahead in a new subject quickly. Our expert authors combine facts, analysis, perspective, new ideas, and enthusiasm to make interesting and challenging topics highly readable.
