Sick sinus syndrome is characterised by persistently slow heart rate, decreased heart rate response to exercise, sudden pauses in the normal activity of the sinusnode (sinus arrest or sinoatrial block) and occasional episodes of nodal or atrial escape rhythm. The syndrome refers to a disease affecting the sinus node itself and not to a disturbed regulation of the functioning of the node as is seen in reflex bradycardia. It is usually seen in association with other cardiac problems in elderly patients. The symptoms are brought on by the slowness of the heart rate (breathlessness and other symptoms suggestive of heart failure) or the sinus pauses (presyncope or syncope). Atrial fibrillation is also common as are other atrial tachyarrhythmias (bradycardia-tachycardia syndrome). In these patients, long sinus pauses are often noted following the discontinuation of the atrial arrhythmia.

For the clinician the sinus node is more or less a hidden structure and only by indirect assessment he is able to say something about the function of this center of pacemaker activity. The morphologist, however, is able to describe the structural microscopic and even electronmicroscopic features of this structure. The only disadvantage is that, as soon as he is coming into the picture, the tissue is dead. The physiologist tries to investigate the electrophysiological behavior of the sinus node. Since there is not a human being willing to give his sinus node for research, he has to do his investigation on isolated preparations of animals. Though there are a lot of experts in the field of the sinus node they nearly never speak the same 'language'. Therefore, it was my dream to bring all those people - or at least some of them -together on a workshop. I had the feeling that it was important to organize such a meeting without an audience but only with 'experts'.

Regional anesthesia decreases the risk of respiratory complications and postoperative delirium/dementia in this predominantly elderly population with multiple comorbidities. However, spinal anesthesia may result in the manifestation of sick sinus syndrome, which occurs mainly in older adults.We discuss a patient with undeclared sick sinus syndrome who presented after spinal anesthesia. Case reportAn 89-year-old gentleman with a past medical history of dementia, hypertension, hyperlipidemia and hearing impairment was planned for right intertrochanteric fracture fixation. An uncomplicated spinal anesthesia was given (2.5ml heavy bupivacaine 0.5% + 10mcg fentanyl at L4/5 level). However, upon transfer to the operating theatre, the patient became progressively bradycardic and unresponsive. He was resuscitated and sent to ICU. A diagnosis of sick sinus syndrome was subsequently made. DiscussionSick sinus syndrome (SSS) comprises a variety of conditions involving sinus node dysfunction, resulting in the inability to generate a physiologically appropriate heart rate. The sudden sympathetic block with spinal anesthesia gives little time for cardiovascular compensation, especially in patients with SSS who have low physiologic reserve. Block height determines the extent of sympathetic blockade, which may vary between two to six dermatomes above the sensory level and incompletely below this level. Although lower limb surgery under regional anesthesia may benefit a large population of elderly patients, central neuroaxial blockade should be avoided if any suspicion of SSS is present.
