"Vasovagal Syncope (VVS) is defined as a transient loss of consciousness with spontaneous and full remission. VVS affects a substantial proportion of the population and is associated with increased morbidity, reduced quality of life and significant impairment. VVS also adds to the economic global burden of disease. Although significant research has focused on understanding the pathophysiology of VVS, the interplay between certain factors remains unclear. This thesis aims to examine some of these poorly understood associations. The first study carefully examined the hemodynamic mechanisms leading to VVS during a head-up tilt (HUT) table test, a postural stress test designed to diagnose and evaluate VVS. Results suggest a more nuanced classification of VVS based on these processes. The second study examined the relationship between real-life triggers and the hemodynamic processes involved in VVS. Although some differences between clinical groups in terms of physiological changes during VVS were identified, many similarities were also observed. Finally, the third study assessed the presence and onset of electrodermal activity (EDA), an indirect measure of sweating, in relation to both clinical triggers and hemodynamic mechanisms of VVS. Most patients who experienced presyncope during HUT testing exhibited EDA prior to presyncope. EDA was more likely to occur upon initial HUT in patients with an emotional history of VVS, and at the end of HUT in patients who experienced large vasodilatory responses leading to VVS. Overall, the pathophysiology of VVS is complex, and real-life triggers, physiological processes and premonitory symptoms should be evaluated and integrated into the understanding of this diagnosis. " --
