The enormous individual, familial, and societal burden of child sexual abuse has underscored the need to address the problem from a public health framework. Much work remains, however, at the first step of this framework--defining and understanding the scope of the problem, or establishing incidence and prevalence estimates. In this occasional paper, we provide an overview of the ways researchers have defined and estimated the scope of child sexual abuse, focusing on agency tabulations and large-scale surveys conducted over the last several decades. More precise estimates of the number of children affected by child sexual abuse would improve the ability of the public health, child welfare, pediatrics, and other communities to prevent and respond to the problem. We recommend using a comprehensive surveillance system to assess and track the scope of child sexual abuse. This system should be grounded by common definitional elements and draw from multiple indicators and sources to estimate the prevalence of a range of sexually abusive experiences.

Child Abuse as a social problem in the Caribbean has only within the past thirty years gained recognition and attention since the emphasis was placed on the more prevalent maternal and child health problems. The phenomenon of child abuse and neglect, however, is not clearly defined due to inadequate surveillance systems within the territories. Studies based on reported cases have established that there are traditional child rearing practices which adversely affect some Caribbean children due to many factors ranging from poverty, inequality, oppression, gender issues, socio-economic disadvantages and loss of values. Because of these deficiencies, various forms of child abuse have been identified particularly in the urban areas which may be considered the outcome of abnormalities in the parent /child relationship and of the society. The Book presents a comprehensive portrayal of the phenomenon of child abuse and neglect in Caribbean society drawing on research and clinical experience. It incorporates history, case scenarios and a variety of effective intervention strategies. In addition, it explains case management procedures focusing on how various professions can become involved in the child protection process; and the need to convey in a different light the changes that need to occur to break the cycle of child abuse and neglect. The Book is written for professionals who work in the best interest of children on a daily basis and intended to offer a good basic introduction to the topic. It is a useful tool for therapists, educators, students, social workers, nurses, police officers. It is intended to stimulate those who read it not only to improve their day-to-day practice in responding to child abuse but will also broaden their perspective about child abuse and neglect. The Book also explains how the needs of children and the relationship between them and their parents are predictive of child abuse and neglect; and examines its nature, causes, recognition and response, its effects on children, the legislation and the social and other obstacles to effective implementation of the Law. The author draws attention to the fact that the majority of this study focuses primarily on people of African descent since little is known about children in other ethnic groups.

Child abuse policy in the United States contains dangerous contradictions, which have only intensified as the public slowly accepted it as a middle class problem. One contradiction is the rapidly expanding child abuse industry (made up of enterprising psychotherapists and attorneys) which is consuming enormous resources, while thousands of poor children are seriously injured or killed, many while being "protected" by public agencies. This "rediscovery" has also led to the frenzied pursuit of offenders, resulting in the sacrifice of some innocent people. Moreover, the media's focus on the sensational details of high-visibility sexual abuse cases has helped to trivialize, if not commercialize, the child abuse problem. As such, child abuse has gone from a social problem to a social spectacle. By the 1980s the child welfare system had become a virtual "nonsystem," marked by a staggering turnover of staff, unmanageable caseloads, a severe shortage of funding, and caseloads composed of highly dysfunctional families (many with drug-related problems). To make room for these families, public agencies rationed services by increasingly screening-out child abuse reports which contained little likelihood of serious bodily harm. In The Politics of Child Abuse in America, the authors argue that child abuse must be viewed as a public safety problem. This redefinition would make it congruent with other family-based social trends, including the crackdown on domestic violence. Children must have the same legal protection currently extended to physically and sexually abused women. This can be done by creating a "Children's Authority," which would have the overall charge for protecting children. Specifically, Children's Authorities would have the responsibility for providing the six main functions of child protection: investigation, enforcement, placement services, prevention and education, family support, and research and development. Offering a unique perspective on the cold reality of this crisis, The Politics of Child Abuse in America will be a provocative work for social workers and human service personnel, as well as the general reader concerned with this timely issue.

This book is about men and women who neglect and abuse children, physically, emotionally and sexually. A number of key themes emerge: Emotional abuse causes the most long-term harm to children, although combinations of emotional with physical and/or sexual abuse cause the most harm to long-term mental health. Because of the long-term burden which abuse imposes on its victims, it is crucial to intervene effectively with abusers (potential and actual) to prevent them from either beginning abuse, committing further abuse, or entering the victim-to-abuser cycle. Abusers have often experienced physical, emotional or sexual abuse or neglect in their own childhoods. The role of disordered attachments to adult figures in the victim-to-abuser cycle is considered in detail, including ways of effective intervention. Men and women who kill children can be characterized in various ways, and various programmes of intervention can be designed. The cycle of poverty is also considered in detail, a process by which economically poor families beget children who enter a cycle of both economic and psychological poverty, in which risks of all kinds of child abuse and neglect are elevated.
