Successful theatrical productions are a team effort and require the close cooperation of the playwright, producer, director, designers, and actors. The group responsible for selecting a play and the style of its production must first reach a consensus on their reason for being and their rationale for approaching an audience. The goals and modes of production are constantly evolving, requiring theatre personnel to be constantly conversant with shifts in the functions of members of theatre teams, in forms and styles of drama, and in techniques of staging. This book stresses the need for collaboration and communication among the members of the theatre team during the moving of a script toward its audience. Though evolution in the roles of producer, playwright, and director has been neither uniform nor evenly paced, this book demonstrates that change itself provides theatre teams openings for inspiration and creation. Through examples of production successes and failures of eminent plays since mid-century, and through discussions of specific interaction or lack of it among those who produced and directed the plays, this volume stresses clearly delegated authority and responsibility of production roles. Full-scale interaction is vital as the members of the theatre team interpret, rehearse, and perform a play. This book also includes sections on the different production circumstances encountered by theatre teams of various levels and excerpts from interviews with theatre professionals.

The World Health Organisation (WHO) introduced the Surgical Safety Checklist (SSC) to change practice in surgical theatres, foster better team communication and to encourage a culture of safety. Despite the SSC including a point where team members introduce themselves by name and role subsequent recollection of this information may be difficult especially in an emergency situation. This project looked at the ability of theatre team members to identify the name and role of their colleagues during routine day case surgery.Materials and Methods: During the period September to October 2011 staff in operating theatres at Queen Alexandra Hospital, Portsmouth were asked to identify the names and roles of other members of the operating team. Theatre staff were surveyed before and after a whiteboard was installed for recording team members' names and roles. Team members were asked to identify their colleagues without referring to the white board.Results and Discussion: During the two month period 160 members of operating theatre teams were asked to identify their colleagues. In theatres where no white board was present to record team members names 64/80 respondents correctly identify their colleagues (80%, 0.95 confidence interval (CI): 70-87%) see figure 1. In theatres where team names and roles were recorded on a white board this rose to 80/80 team members being correctly identified (100%, 0.95 CI: 96-100%).The addition of a white board on the wall of the operating theatre led to a marked improvement in name and role identification. This improvement may reflect increased recollection of the original team briefing process.Conclusion(s): In both routine and emergency situations the ability to communicate effectively within the team is integral to the safety of the patient. Knowing the names and roles of other members of the team is fundamental to the WHO SSC and to building successful, safe operating theatre teams. Future work could focus on whether improved identification of team members in a routine operating theatre environment results in improved patient outcomes.

Operating theatres are very private workplaces. There have been few research investigations into how highly trained doctors and nurses work together to achieve safe and efficient anaesthesia and surgery. Confronted with worldwide evidence of substantial public health harm due to inadequate patient safety, the World Health Assembly (WHA) in 2002 adopted a resolution (WHA55.18) urging countries to strengthen the safety of health care and monitoring systems. The resolution also requested that WHO take a lead in setting global norms and standards and supporting country efforts in preparing patient safety policies and practices. In May 2004, the WHA approved the creation of an international alliance to improve patient safety globally; WHO Patient Safety was launched the following October. For the first time, heads of agencies, policy-makers and patient groups from around the world came together to advance attainment of the goal of "First, do no harm" and to reduce the adverse consequences of unsafe health care. The purpose of WHO Patient Safety is to facilitate patient safety policy and practice. It is concentrating its actions on focused safety campaigns called Global Patient Safety Challenges, coordinating Patients for Patient Safety, developing a standard taxonomy, designing tools for research policy and assessment, identifying solutions for patient safety, and developing reporting and learning initiatives aimed at producing 'best practice' guidelines. Together these efforts could save millions of lives by improving basic health care and halting the diversion of resources from other productive uses. The Global Patient Safety Challenge, brings together the expertise of specialists to improve the safety of care. The area chosen for the first Challenge in 2005-2006, was infection associated with health care. This campaign established simple, clear standards for hand hygiene, an educational campaign and WHO's first Guidelines on Hand Hygiene in Health Care. The problem area selected for the second Global Patient Safety Challenge, in 2007-2008, was the safety of surgical care. Preparation of these Guidelines for Safe Surgery followed the steps recommended by WHO. The groundwork for the project began in autumn 2006 and included an international consultation meeting held in January 2007 attended by experts from around the world. Following this meeting, expert working groups were created to systematically review the available scientific evidence, to write the guidelines document and to facilitate discussion among the working group members in order to formulate the recommendations. A steering group consisting of the Programme Lead, project team members and the chairs of the four working groups, signed off on the content and recommendations in the guidelines document. Nearly 100 international experts contributed to the document (see end). The guidelines were pilot tested in each of the six WHO regions--an essential part of the Challenge--to obtain local information on the resources required to comply with the recommendations and information on the feasibility, validity, reliability and cost-effectiveness of the interventions.

Subsequent debriefing of these results identified that theatre staff do not get feedback on patient experience, outcomes or team functioning. An understanding and appreciation of the high quality care that perioperative teams deliver is crucial to provide meaning in work, job satisfaction and to helpprevent burnout amongst theatre staff. We hypothesised that improving feedback on patient experience to perioperative teams would improve staff morale, job satisfaction and reduce burnout, and with the potential to improve patient outcomes.Materials and Methods: Building on pilot work in one theatre, 4 medical students will lead the development and spread of feedback to theatre staff of perioperative patient experience. The pilot started with paper feedback with a plan to develop a computer system that could be used by patient volunteers to collect the feedback. This will be fed back to the theatre staff. The frequency and the exact method of feedback needs to be determined through iterative tests of change.Results and Discussion: At baseline theatre staff reported that 67% of them believed that morale impacted on patient care, 75% wished to know more about patient experience, 100% that getting feedback on patient experience would influence their practice and the effectiveness of the team, 83% that knowing more about patient outcomes would improve job satisfaction. 25% of staff wished to receive the feedback weekly and 75% of staff wished to receive it monthly. No staff wished to receive it on a daily basis. The frequency and best method of feedback will be tested by iterative tests of change.Conclusion(s): Feedback to operating theatre staff on perioperative patient experience has the potential to improve staff morale, job satisfaction and decrease burnout and to improve patient experience and outcomes. Further work is required to test the use of iPads and patient volunteers to collect the perioperative feedback. Institutional inertia can provide a barrier to improvement.The process has been successful in introducing quality improvement methodology to undergraduate medical students.
