This book is a research mono graph reporting empirical results, but we have tried to place the data in a very broad national perspective. Our intent is a volume on mental health policy in the United States, most notably our de facto policies, as indicated by empirical data. The book gives a broad perspective of mental disorders and mental disorder treatment in general hospitals in the United States. The history of psychiatry is complex, reflecting diverse origins in mythology, cult beliefs, astrology, early medicine, law religion, philosophy, and politics. This complexity has generated considerable debate and an increasing outflow of historical scholarship, ranging from the enthusiastic meliorism of pre-World War II histories, to the iconoclastic revisionism of the 1960s, to more focused studies, such as the history of asylums and the validity and efficacy of Freudian theory. This volume, intended as a successor to the centennial history of American psychiatry published by the American Psychiatric Association in 1944, summarizes the significant events and processes of the half-century following World War II. Most of this history is written by clinicians who were central figures in it. In broad terms, the history of psychiatry after the war can be viewed as the story of a cycling sequence, shifting from a predominantly biological to a psychodynamic perspective and back again -- all presumably en route to an ultimate view that is truly integrated -- and interacting all the while with public perceptions, expectations, exasperations, and disappointments. In six sections, Drs. Roy Menninger and John Nemiah and their colleagues cover both the continuities and the dramatic changes of this period. The first four sections of the book are roughly chronological. The first section focuses on the war and its impact on psychiatry; the second reviews postwar growth of the field (psychoanalysis and psychotherapy, psychiatric education, and psychosomatic medicine); the third recounts the rise of scientific empiricism (biological psychiatry and nosology); and the fourth discusses public attitudes and perceptions of public mental health policy, deinstitutionalization, antipsychiatry, the consumer movement, and managed care. The fifth section examines the development of specialization and differentiation, exemplified by child and adolescent psychiatry, geriatric psychiatry, addiction psychiatry, and forensic psychiatry. The concluding section examines ethics, and women and minorities in psychiatry. Anyone interested in psychiatry will find this book a fascinating read.

While insights sometimes are slow in coming, they often seem obvious when they finally arrive. This handbook is an outcome of the insight that the topics of social support and the family are very closely linked. Obvious as this might seem, the fact remains that the literatures dealing with social support and the family have been deceptively separate and distinct. For example, work on social support began in the 1970s with the accumulation of evidence that social ties and social integration play important roles in health and personal adjustment. Even though family members are often the key social supporters of individuals, relatively little re search of social support was targeted on family interactions as a path to specifying supporter processes. It is now recognized that one of the most important features of the family is its role in providing the individual with a source of support and acceptance. Fortunately, in recen t years, the distinctness and separateness of the fields of social support and the family have blurred. This handbook provides the first collation and integration of social support and family research. This integration calls for specifying processes (such as the cognitions associated with poor support availability and unrewarding faIllily constellations) and factors (such as cultural differences in family life and support provision) that are pertinent to integration.

Several people have asked what motivated us to write a book about commut ing, something that we all do but over which we have very little control. As a matter of fact, the general reaction from professional colleagues and friends alike was first a sort of knowing smile followed by some story. Everyone has a story about a personal commuting experience. Whether it was a problem with a delayed bus, a late arrival, broken-down automobiles, hot trains or subways, during the past year we have heard it all. Many of these stories must be apocryphal because, if they were all true, it is amazing that anyone ever arrived at work on time, at home, or at some other destination. The interest for us likely stems from many factors that over the years have probably influenced our thinking. All of the authors studied and/or grew up in the New York City metropolitan area. For illustration, let's devote a few paragraphs to describing some of the senior author's (Koslowsky's) life experiences. As a young man in New York City, he was a constant user of the New York City subway system. The whole network was and still is quite impressive. For a relatively small sum, one can spend the whole day and night in an underground world (growing up in New York often makes one think that the whole world is contained in its five boroughs).

The nosological roots of post-traumatic stress disorder (PTSD) may be traced back to th~American Psychiatric Association's DSM-I entry of gross stress reaction, as published in 1952. Yet the origins of the current enthusi asm with regard to post-traumatic stress can be traced back to 1980, which marked the emergence of the term post-traumatic stress disorder in the DSM III. This reflected the American Psychiatric Association's acknowledgment of post-traumatic stress as a discrete, phenomenologically unique, and reli able psychopathological entity at a time in American history when such recognition had important social, political, and psychiatric implications. Clearly, prior to DSM-I the lack of a generally accepted terminology did little to augment the disabling effects that psychological traumatization could engender. Nor did the subsequent provision of an official diagnostic label alone render substantial ameliorative qualities. Nevertheless, the post Vietnam DSM-III recognition of PTSD did herald a dramatic increase in research and clinical discovery. The American Red Cross acknowledged the need to establish disaster mental health services, the American Psychological Association urged its members to form disaster mental health networks, and the Veterans Administration established a national study center for PTSD.

I n the wake of an earlier book (Solomon, 1993), this new work, Coping with War-Induced Stress: The Gulf War and the Israeli Response, promises to make Zahava Solomon a modern maven with respect to the psychologi cal effects of war. Dr. Solomon is a high-ranking officer, serving as a psychiatric epidemiologist in the Mental Health Department of the Is raeli Defense Forces Medical Corps. She also teaches at Tel Aviv Univer sity. The earlier book dealt with the reactions of the Israeli Defense Forces to the 1982 war in Lebanon, which divided the population of Israel concerning its wisdom and justification. The new book deals with the emotional consequences of the United Nations effort against Iraq after its invasion of Kuwait. Because Israel agreed not to participate actively so as not to endanger the fragile Arab coalition against Iraq, it was in a sense a nonwar-as Solomon refers to it-yet with many fea tures of a war. Although they had quite limited casualties, largely in the Tel Aviv area, the Israelis faced the actuality of damaging Scud missile attacks and the threat that these missiles could not only be targeted to much of Israel but also carry poison gas to other Israeli cities. Solomon has written a fascinating book about this crisis in Israeli life.

Much of what we know about the subject of coping is based on human behavior and cognition during times of crisis and transition. Yet the alarms and m~or upheavals of life comprise only a portion of those experiences that call for adaptive efforts. These chronic stressors issue in part from persistently difficult life circumstances, roles, and burdens, and in part from the conversion of traumatic events into persisting adjustment challenges.
