Alcohol abuse and alcoholism represents one of the major health, social, and economic issues facing not only America but much of the world. Problems with alcohol appear to be a common curse, afflicting almost all industrialized nations as well as the developing countries. at least 10 million people suffer the misuse. It is In the United States today of estimated that 205,000 individuals die prematurely each year from a variety ethanol-induced factors, including cirrhosis, cancer, heart disease, suicide, homi cide, and highway fatalities and other accidents. In purely economic terms, the alcohol-related cost to our society in 1975 is estimated at nearly $43 billion in lost production, medical expenses, motor vehicle accidents, fire losses, and the maintenance of social mechanisms to deal with these problems. But the economic impact pales in comparision with the psychological pain and anguish brought to alcoholics and their family and friends. The disease of alcoholism bears a close relation to the topic of our dedication, for it is equally insidious and sadistic, it destroys humanity and rational thought, and it brings terror, pain, and death.

First published in 1992, Fundamentals of Psychiatric Treatment Planning outlines an approach that quickly became the definitive standard for writing treatment plans. Developed by clinical psychiatrist James A. Kennedy, this practical, intuitive method organizes psychiatric problems into seven categories: psychological impairment, social skills, violence, activities of daily living, substance abuse, medical impairment, and ancillary impairment. Treatment plans are developed using information gathered with the Kennedy Axis V, an instrument that has proven more successful than both the Global Assessment of Functioning (GAF) scale and the Brief Psychiatric Rating Scale (BPRS) in establishing baselines and determining outcome measures. Today, most substance abuse treatment is administered by community-based organizations. If providers could readily incorporate the most recent advances in understanding the mechanisms of addiction and treatment, the treatment would be much more effective and efficient. The gap between research findings and everyday treatment practice represents an enormous missed opportunity at this exciting time in this field. Informed by real-life experiences in addiction treatment including workshops and site visits, Bridging the Gap Between Practice and Research examines why research remains remote from treatment and makes specific recommendations to community providers, federal and state agencies, and other decision-makers. The book outlines concrete strategies for building and disseminating knowledge about addiction; for linking research, policy development, and everyday treatment implementation; and for helping drug treatment consumers become more informed advocates. In candid language, the committee discusses the policy barriers and the human attitudesâ€"the stigma, suspicion, and skepticismâ€"that often hinder progress in addiction treatment. The book identifies the obstacles to effective collaboration among the research, treatment, and policy sectors; evaluates models to address these barriers; and looks in detail at the issue from the perspective of the community-based provider and the researcher.

This book focuses on the value of supervised Disulfiram therapy, highlighting the many potential and unique benefits of Disulfiram. One of the oldest drugs available for the long-term management of alcohol dependence, Disulfiram remains a viable treatment option for alcohol dependence and has been shown in recent studies to be more successful in treating patients with alcohol dependence than Naltrexone, Topiramate and Acamprosate. It is also useful in dual diagnosis patients and those with co-morbid cocaine and alcohol dependence. Although Disulfiram’s mechanism of action in alcohol dependence was long thought to be its effects as a psychological deterrent, more recent studies point to potential anti-craving effects as well. In dedicated chapters, the book reviews major clinical trials of Disulfiram spanning nearly 60 years, its historical aspects and discovery, side effects, treatment protocols and uses in the context of alcohol dependence. The book also discusses the use of Disulfiram across diverse populations along with monitoring for compliance and various adverse effects that may manifest. Further topics include Disulfiram implant therapy and the role of Disulfiram in the modern long-term pharmacotherapy of alcohol dependence, as well as the role of cognitive behavior therapy in enhancing the effects of Disulfiram and the emerging role of Disulfiram in treating cocaine dependence and pathological gambling. As such, the book offers a “one-stop” comprehensive guide to all aspects of Disulfiram therapy.

The opioid crisis in the United States has come about because of excessive use of these drugs for both legal and illicit purposes and unprecedented levels of consequent opioid use disorder (OUD). More than 2 million people in the United States are estimated to have OUD, which is caused by prolonged use of prescription opioids, heroin, or other illicit opioids. OUD is a life-threatening condition associated with a 20-fold greater risk of early death due to overdose, infectious diseases, trauma, and suicide. Mortality related to OUD continues to escalate as this public health crisis gathers momentum across the country, with opioid overdoses killing more than 47,000 people in 2017 in the United States. Efforts to date have made no real headway in stemming this crisis, in large part because tools that already existâ€"like evidence-based medicationsâ€"are not being deployed to maximum impact. To support the dissemination of accurate patient-focused information about treatments for addiction, and to help provide scientific solutions to the current opioid crisis, this report studies the evidence base on medication assisted treatment (MAT) for OUD. It examines available evidence on the range of parameters and circumstances in which MAT can be effectively delivered and identifies additional research needed.

Clinical Guide To Alcohol Treatment written by Robert J. Meyers and has been published by Guilford Press this book supported file pdf, txt, epub, kindle and other format this book has been release on 1995-08-04 with Psychology categories.

This book is the first complete guide to implementing the Community Reinforcement Approach (CRA), an empirically based, highly effective cognitive-behavioral program for treating alcohol problems. CRA acknowledges the powerful role of environmental contingencies in encouraging or discouraging drinking, and attempts to rearrange these contingencies so that a non-drinking lifestyle is more rewarding than a drinking one. Unique in its breadth, the approach utilizes social, recreational, familial, and vocational strategies to aid clients in the recovery process. This authoritative manual is a hands-on guide to applying these therapeutic procedures. The authors present a step-by-step guide to each component of the treatment plan, many of which have been shown to be effective forms of treatment in themselves. Topics include behavioral skills training, social and recreational counseling, marital therapy, motivational enhancement, job counseling, and relapse prevention. Each chapter provides detailed instructions for conducting a procedure, describes what difficulties to expect, and presents strategies for overcoming them. Sample dialogues between clients and therapists, annotated by the authors, further illuminate the treatment process. The book concludes with a chapter that both addresses the common mistakes made when implementing CRA, and emphasizes the flexibility and benefits of this total treatment plan. This book is an invaluable resource for a wide range of practitioners including psychologists, psychiatrists, substance abuse counselors, and social workers.
