Disturbances in peripheral O extraction can be produced in dogs treated with 2 endotoxin and thereby provide an opportunity to test theories for the origin of pathological O supply dependency or to try different treatment modalities. The 2 most serious deficiency in the current animal models is the inability to mimic the increased O demand that is observed in patients at 02 delivery rates in excess of 2 normal. A particular feature of this increased O demand is that it apparently does 2 not stimulate increased 02 extraction, although the limitation in O extraction has 2 not been explored in patients by lowering 02 supply, for obvious reasons. At least two possibilities to account for increased 02 demand could be investigated in animal models, however. The amount of 02 that is utilized in extramitochondrial pathways, which is normally on the order of 10%, may be greatly increased in ARDS and sepsis by O radical formation. There is presently no information 2 concerning how much 02 might be used in this way. Another strong possibility is that mitochondrial injury, perhaps as a result of 02 radical formation, uncouples oxidative phosphorylation. Some evidence presently in the literature supports this idea [19]. Indeed, the association of increased blood lactate levels with higher than expected 02 demands makes uncoupling a very attractive hypothesis that warrants further investigation in animal models using such agents as 2,4-dinitrophenol. This presentation describes various aspects of the regulation of tissue oxygenation, including the roles of the circulatory system, respiratory system, and blood, the carrier of oxygen within these components of the cardiorespiratory system. The respiratory system takes oxygen from the atmosphere and transports it by diffusion from the air in the alveoli to the blood flowing through the pulmonary capillaries. The cardiovascular system then moves the oxygenated blood from the heart to the microcirculation of the various organs by convection, where oxygen is released from hemoglobin in the red blood cells and moves to the parenchymal cells of each tissue by diffusion. Oxygen that has diffused into cells is then utilized in the mitochondria to produce adenosine triphosphate (ATP), the energy currency of all cells. The mitochondria are able to produce ATP until the oxygen tension or PO2 on the cell surface falls to a critical level of about 4–5 mm Hg. Thus, in order to meet the energetic needs of cells, it is important to maintain a continuous supply of oxygen to the mitochondria at or above the critical PO2 . In order to accomplish this desired outcome, the cardiorespiratory system, including the blood, must be capable of regulation to ensure survival of all tissues under a wide range of circumstances. The purpose of this presentation is to provide basic information about the operation and regulation of the cardiovascular and respiratory systems, as well as the properties of the blood and parenchymal cells, so that a fundamental understanding of the regulation of tissue oxygenation is achieved.

The International Society on Oxygen Transport to Tissue (ISOTT) was founded in 1973 "to facilitate the exchange of scientific information among those interested in any aspect of the transport and/or utilization of oxygen in tissues". Its members span virtually all disciplines, extending from various branches of clinical medicine such as anesthesiology, ophthalmology and surgery through the basic medical sciences of physiology and'biochemistry to most branches of the physical sciences and engineering. The seventeenth annual meeting of ISOTT was held in 1989 for four days, from July 21 to 24, at the Max Planck Institute for Experimental Medicine and the adjoining University Hospital (Klinikum), in Goettingen, Federal Republic of Germany. It attracted 147 active registrants and approximately 40 accompanying persons. The very successful format originated by Dr. Ian Longmuir in 1985, consisting of posters accompanied by an abbreviated oral summary, was continued with slight modification. Virtually all of the presentations utilized this format, with each poster session preceded by a formal discussion during which the presenter briefly reviewed the poster aided by a few slides. All posters remained in place for the entire four days of the meeting.

The International Society on Oxygen Transport to Tissue (ISOTT) was founded in 1973 "to facilitate the exchange of scientific information among those interested in any aspect of the transport and/or utilization of oxygen in tissues". Its members span virtually all disciplines, ex tending from various branches of clinical medicine such as anesthesiology, ophthalmology and surgery through the basic medical sciences of physiology and biochemistry to the physical sciences and engineering. The fifteenth annual meeting of ISOTT was held in 1987 for three days, from July 22 to 24, at Hokkaido University in Sapporo, Japan. Previously, all ISOTT meetings had been held in Europe or the USA alternatively. This time, however, the meeting was held for the first time in an Asian country. When we first started preparing for this meeting some of our members were afraid that the number of those attending would not exceed '30. Fortunately the results were quite different. We had more than 60 participants from abroad and an even greater number from Japan. In addition to three special lectures and two symposia there were a total of 88 posters presented over the three days of the meeting. These covered all aspects of physiological oxygen transport including convection, diffusion, chemical reaction, and control of oxygen demand in blood and various tissues as well as the methods, models and instrumentation for their study. The 92 papers which comprise this volume encompass all of these areas.

The International Society on Oxygen Transport to Tissue (IS OTT) was founded in 1973 as a scientific society providing a forum for bioengineers, basic scientists (physiologists, biochemists and physicists) and clinicians (including anesthesiologists, intensive care specialists, pediatricians, neonatologists, internists, surgeons and other specialists) to facilitate the exchange of scientific information among those interested in any aspect of the transport and/or utilization of oxygen in tissues. From the ranks of its members, many fundamental discoveries and inventions have been made involving the many aspects of oxygen transport and utilization by biological tissues. The ISOTT proceed ings, now in its 14th edition, has become a standard work in the field as witnessed by the inclusion in the Science Citation Index of all volumes published so far. The 19th ISOTT Meeting was held in Cura~ao from August 24th through August 30th, 1991. The Cura~ao Meeting attracted 145 registrants and 45 accompanying persons. The format originated by Dr. Ian Llngmuir in 1985, consisting of posters ac companied by an abbreviated oral summary, was again successfully handled with slight modifications. The meeting was introduced by 6 review lectures covering the whole field of oxygen transport from bioengineering, the problem of diffusion in lung, blood and tissue through pathology of oxygen uptake in the lung, oxygen supply dependency of the critically ill to artificial oxygen carriers. A special session dealt with oxygen sup ply under ambiant pressure changes.

The International Society on OXygen Transport to Tissue (ISO'IT) has canpleted nine years as a society since its first fonnal meeting at Charleston-Clanson, South Carolina, United States of America in 1973. Prior to this time an active group of scientists and engineers rret in w::>rkshop environrrents, on a periodic basis, around the w::>rld. Meetings are ncM on an annual basis, alternating between Europe and the United States. The international gatherings include scientists and engineers in a variety of fields. ISOIT has produced a stable forum for the exchange of info:rmation on the rnicroenvirornnent of living cells, ranging fran the utilization of mathematics and engineering, through physiology and radiobiology, to clinical applications. The proceedings of these meetings have been codified into six books, four of them published by Plenum Press in its prestigious Advances in Experimental Medicine and Biology series. This volume, together with the next two volumes that will follCJV.l the meetings in Dortmund, Gennany in 1982 and in Ruston, Louisiana in 1983 will provide further chapters in the history of this fascinating field of knCJV.lledge. OUr thanks are given to all the participants and contributors to the Detroit meeting. We hope that the strength of the society will grow in caning years, and that our contribution will eventually be felt in :inproving the treatment of the sick, and in enhancing the thought processes of the intellectual.

The ISOTT 2001 local organizing committee was pleased to welcome over 140 delegates from around the world to the 29th annual general meeting of the International Society for Oxygen Transport to Tissue. The meeting was held in historic Philadelphia, USA, on the campus of the University of Pennsylvania from August 11 to 15, 2001. In the tradition of ISOTT, the conference was a total immersion experience. Attendees were encouraged to eat together and spend their evenings relaxing together in a style that maximized exchange of ideas and interactions of younger scientists with their more senior colleagues. Delegates participated in a total of 122 presentations including poster displays, selected oral presentations, seminars by invited speakers and a round table discussion. In choosing invited speakers and oral presenters, special emphasis was placed on methods for oxygen measurement in living tissue and application of these technologies to understanding physiological and biochemical basis for pathology related to tissue oxygenation. All of the manuscripts contained in this volume underwent both an editorial and scientific review, and only those meeting both criteria have been published. However, while all efforts have been made to eliminate editorial errors, some have undoubtedly been overlooked, for which the editors apologize.

Since its inception in 1973, The International Society on Oxygen Transport to Tissue (ISOTT) has provided a unique forum to facilitate and encourage scientific interaction and debate. Welcoming scientists and clinicians from a broad spectrum of disciplines, each with their own particular skills and expertise, ISOTT unites them under the common theme of oxygen transport. The successful blend of scientific presentations and informal discussion which characterizes ISOTT is epitomized best by the many fundamental discoveries and technical advancements which it has spawned. The breadth and strengths of The Society's scientific base promotes the rapid progression of ideas from theoretical concepts to rigorous scientific testing and often, ultimately to the clinical arena. Each publication of the ISOTT proceedings has been recognized by Science Citation Index listing and the papers frequently establish scientific precedents and become considered as standard works in their respective fields. The 21st ISOTT Meeting was held in San Diego from August 14th through August 18th, 1993. The San Diego Meeting attracted about 150 registrants and 40 accompanying persons. Ten state-of-the-art lectures were presented by international experts in ~ transport and there were in addition two symposia -one dealing with assessment of tissue hypoxia and the other with functional heterogeneity in different organ systems. There were 100 free communications, consisting of posters accompanied by an abbreviated oral summary. All manuscripts were reviewed by the Editors for form and content, but as is customary for the ISOTT proceedings, rigorous scientific peer review was not undertaken.

The theme of the 1983 annual ISOTT meeting emphasizes a dual scientific approach, utilizing interdisciplinary theoretical and experimental methodology, to unravel the secrets of oxygen transport to tissue. ISOTT is leading the way in the development and application of forefront techniques to allow a more basic understanding of this important physiological phenomena. The Society can be proud of its contributions in helping to advance the gross analysis techniques of the past to the synergistic study of tissue micro areas by sophisticated mathematical and experimental means. There is a tremendous need to continue the pursuit of basic knowledge at the regional tissue level and to move rapidly to the examination of fundamental biochemical reactions at the cellular level. To accomplish this, it will be necessary to take advantage of the most powerful computing machinery presently available and to apply state-of-the-art experimental methodology, such as polarographic, autoradiographic and nuclear magnetic resonance techniques in our studies. Our Society must continue to work hard to push back barriers to enhance our understanding of mechanisms that lead to pathology. Hopefully, our efforts will be significant in helping science to overcome the many dreadful diseases that mankind faces in today's world.
