During the past decade, relatively few new studies have been initiated on the response of different species to high-dose, total-body irradia tion. For information on the LDSO/30d (the dose which produces 50 percent lethality within 30 days), one is generally referred to the older literature (e. g., Bond, Fliedner and Archambeau, 1965). Compari son of experimental data reveals considerable variations in LDSD values even after total-body irradiation with conventional X rays, ranging from 4 to 6 Gy in the monkey, 7.1 to 9 Gy in the rat and from 6.4 to 9 Gy in the mouse (see also Hall, 1978). Part of the discrepancy in the LDso values can possibly be attributed to inadequacies in the dosimetry procedures and exposure arrangements employed. As far as clinical experience is concerned, there is now an appreciable amount of information available about the effect of total body irradiation as a conditioning treatment for bone marrow transplan tation in patients suffering from leukaemia or aplastic anaemia. The results from different centres, including the incidence of complications such as radiation pneumonitis, are considerably different. This can part I y be connected with the application of different radiation sched ules: large single dose versus fractionated or protracted irradiation.

In the late 1940s investigators observed that mice given supralethai doses of total body irradiation were protected by infusion of viable spleen or marrow cells following irradiation, and that this was accomplished by hemopoietic reconsti tution with donor cells as proven using genetic markers. If a similar approach could be applied to humans, it should be possible to treat leukemia patients with any dose of chemoradiotherapy as far as nonmarrow toxicity permitted, and then rescue them by marrow transplantation. Early clinical attempts were gen erally unsuccessful, mostly due to a lack of knowledge of histocompatibility antigens and appropriate supportive care. These areas developed rather quickly during the 1960s, and for almost two decades now clinical marrow transplan tation has been carried out with increasing success. After initially using only bone marrow from HLA indentical siblings, the field has expanded rapidly to incorporate HLA nonidentical related donors, and recently even marrow from unrelated volunteer donors. Furthermore, since for numerous patients who oth erwise could benefit from transplantation a donor cannot be identified, there has been a growing interest in using the patient's own (autologous) bone marrow. Our understanding of the principles of transplantation and our knowledge of the potential risks and benefits have quickly grown. At times it is difficult, however, to decide what is the best option for a given patient.

Acute radiation injury in man has been amply reviewed in the past decades (1-8). Information for those reviews was mainly derived from the Hiroshima and Nagasaki experience (3), from radiation accidents as a con sequence of testing nuclear weapons and the application of nuclear energy (9-13) and from the intentional irradiation of patients, mostly for the treatment of malignancies and/or as conditioning for transplantation (14-20). Further, a large body of data derived from animal experimentation has led to cautious extrapolations to the human situation (14). An elaborate review dealing with early somatic effects of radiation in a man was published by UNSCEAR ) in 1962 (2). It was supplemented in 1969 with a similar document dealing mostly with radiation-induced chromosome aberra tions, the effects of radiation on the nervous system and, radio-active contamination of the environment by nuclear testing (21). In a more recent UNSCEAR report (1972) the main topics were genetic effects, carcinogenesis and effects of radiation on the immune response (22). The early effects of radiation in man were also expertly and comprehensively reviewed in a report by the United States National Research Council in 1967 (23).
