The notion of well-being holds relevance to each 17 of the Sustainable Development Goals (SDGs). Well-being is a major underlying driver of policy coherence across sectors and encourages galvanized action. Advancing societal well-being helps create active, resilient and sustainable communities at local, national and global levels – enabling them to respond to current and emerging health threats, such as coronavirus disease 2019 (COVID-19) and environmental disasters. By its comprehensive nature, well-being unites the health, economic, social and environmental dimensions of the sustainable development agenda, forming a political construct that blends the health and quality of life of people and communities, with concepts of equity and planetary sustainability. Recognizing the call for action the idea implies, the aim of this discussion paper is to illuminate well-being as a policy framework and describe its application in practice to inform the strategic work of the World Health Organization as a central component of the Healthier Populations Framework and the sustainable development agenda.

In a world where many experience unprecedented levels of wellbeing, chronic poverty remains a major concern for many developing countries and the international community. Conventional frameworks for understanding development and poverty have focused on money, commodities and economic growth. This 2007 book challenges these conventional approaches and contributes to a new paradigm for development centred on human wellbeing. Poor people are not defined solely by their poverty and a wellbeing approach provides a better means of understanding how people become and stay poor. It examines three perspectives: ideas of human functioning, capabilities and needs; the analysis of livelihoods and resource use; and research on subjective wellbeing and happiness. A range of international experts from psychology, economics, anthropology, sociology, political science and development evaluate the state-of-the-art in understanding wellbeing from these perspectives. Self-care interventions, including for sexual and reproductive health and rights, are among the most promising new approaches to improving health and well-being, both from a health systems perspective and for the users of these interventions. Self-care interventions should be an adjunct to, rather than a replacement for, direct interaction with the health system. These interventions are increasingly being acknowledged in global initiatives, including to advance primary health care (PHC) and universal health coverage (UHC). When they are accessible, available, acceptable, and affordable, quality self-care interventions hold the promise of advancing the attainment of UHC. Building upon the latest WHO recommendations and good practice statements, this implementation guidance includes key considerations to support the introduction and scale-up of self-care interventions in countries. In implementing the global recommendations on self-care interventions for health and well-being, countries will need to adapt them to the local context, considering the economic conditions and the existing health services and facilities. This guidance is intended to assist with that process and requires interaction with all health systems building blocks to ultimately improve national health systems in terms of efficiency, responsiveness, more equitable health outcomes, and social and financial risk protection. This guidance is relevant for all settings and should, therefore, be considered as global guidance.
