Robotic Surgery of the Head and Neck is the first comprehensive guide for otolaryngologists who wish to perform robotic head and neck surgery. Edited by leaders in the field, this book focuses on how improved access, visualization, and flexibility of the technology have greatly expanded the capabilities of the head and neck surgeon to treat diseases transorally or through small incisions in the skin. Starting with an overview of minimally invasive surgery in the head and neck, and moving to discussions of anatomic considerations for these procedures and the future applications of robotic surgery for otolaryngologists, Robotic Surgery of the Head and Neck explores the exciting progress of robotic technologies, bringing physicians closer to achieving the benefits of traditional surgery with the least amount of disruption to the patient.

We live in an era of increasing availability of highly technological tools that allow for reducing, where possible, the invasiveness of the surgical procedures needed to remove cancer. Also in the head and neck area, transoral procedures increasingly replace the traditional, more invasive, open access procedures. Crucial developments promoting this evolution were the popularization of transoral laser microsurgery and, more recently, transoral robotic approaches. It is logical to expect that these approaches will entail better function for the patients, and thus better quality of life, provided that the obtained oncological radicality is the same or better than that obtained with the more invasive procedures of the past. All these “logical” assumptions still need confirmation and validation on large and independent series, however. This eBook collects the results of some of the most recent research efforts in this field. It contains studies aimed at advancing surgical technology and oncological philosophy, as well as studies assessing the oncological as well as the functional outcome following the application of these techniques. In this way this e-book aims at critically assessing and, where justified, boosting the development and popularization of these transoral approaches. The overall aim is evidently to improve both the oncological as well as the functional outcomes for the patient with laryngeal cancer.

Even the very concept of Robotic Surgery is changing. We are developing Computer Assisted Robotic Surgical Systems (CASS) and certainly, at some point, some steps will begin to be automated with a direct intervention of the machine on the patient. This will depend basically on warranties that patient safety is preserved. We are still not there (but on the path).Training is paramount. We conducted our first dissection course on Robotic Surgery in February 2015. Given the accelerated pace of the technical and surgical evolution the need for updates is constant. We are confident that the present text will be useful as a guide for experimental training in Robotic Surgery. We have included all the basic contents of the regular training path (including an extensive discussion on how to drive the da Vinci robotic surgery system), as well as the two current main applications in the head and neck: TransOral Robotic Surgery (TORS) and Remote Access to the neck (robotic thyroidectomy and beyond). Head and Neck Surgeons interested in Robotic Surgery might find valuable tips and practical information. But also, for those who just want to know what is all this about and what the current situation is, we have tried to transmit a plain and honest view coming directly from the original authors from whom we have learnt.Summary.1. Robotic Surgery in Otolaryngology, Head & Neck Surgery demonstrates the advantages of robotic surgery, providing guidance on accessing parts of the head and neck which are difficult to visualise, and are manually inaccessible to the surgeon. The book is divided into thirteen chapters across four sections. The first section covers transoral robotic surgery (TORS), a procedure to remove oral cancers whereby a surgeon uses a sophisticated, computer-enhanced system to guide the surgical tools (mayoclinic.org). The second section provides information on robotic thyroid surgery. The third section covers head and neck reconstruction using TORS and the final section discusses forms of robotic surgery currently in development. Robotic Surgery in Otolaryngology, Head & Neck Surgery is a forward-thinking book, written by leading practitioners and edited by US based specialist Nilesh R Vasan from the University of Oklahoma Health Sciences Centre. With nearly 200 full colour images and illustrations, including many diagrams, this is an invaluable guide to current and future technologies in robotic surgery for otolaryngologists, and head and neck surgeons. Over the last decades, oropharyngeal cancer treatment has changed due to the introduction of new surgical techniques such as transoral approaches. These new techniques provide oncological results that are comparable to previous treatment but with shorter hospital stays and the preservation of functions. This surgery makes necessary to understand the anatomy of the upper aerodigestive pathway from the inside-out, as opposed to the inverse, which is the way head and neck surgeons have classically understood it. Although descriptions of the anatomy from medial to lateral have been published, a clear anatomic stratification of this area should help in the planning and execution of this surgery. This book is the basic guide for an anatomical course with the purpose to identify anatomic landmarks in the oropharynx and base of the tongue. This can help surgeons identify neurovascular elements of the surgical field during transoral robotic surgery quickly and accurately.
