The pharmacological fight against epilepsy began many centuries ago when Hippocrates discovered that the cause of epilepsy is natural, as opposed to supernatural and, as a consequence, must be treated with a natural remedy. Entre un 20 y un 40% de todas las Epilepsias del Lóbulo Temporal (ELT) se comportan como Epilepsias Refractarias a tratamiento médico (ER) [Cockerell et al, 1997; Kwan & Sander, 2004], según la definición de la Liga Internacional Contra la Epilepsia [Kwan et al, 2010]. Estos enfermos son subsidiarios de alternativas terapéuticas no farmacológicas como la cirugía de la epilepsia, que ha demostrado su capacidad para conseguir la remisión completa de las crisis en un porcentaje considerable de estos casos [Spencer & Hugh, 2008]. La demora de la cirugía tiene un efecto deletéreo en la calidad de vida del enfermo epiléptico. Sin embargo, actualmente, los enfermos que acceder a recursos quirúrgicos, no lo hacen antes de los 20 años de media evolución de su enfermedad [Engel et al. 2003]. En buena parte eso es debido a que no existe en la actualidad un instrumento capaz de adelantar con fiabilidad el diagnóstico de refractariedad antes de los dos años establecidos en la definición operativa de ER en función de las características clínicas y paraclínicas de los pacientes. OBJETIVOS: 1.- Describir las características clínicas y paraclínicas de la ELT del adulto en nuestro medio. 2.- Describir las variables clínicas y paraclínicas asociadas con refractariedad en la ELT del adulto y realizar una estimación del riesgo. 3.- Establecer y validar un modelo de predicción de refractariedad basado en datos semiológicos y de pruebas complementarias. DISEÑO DEL ESTUDIO: Parte 1.- Estudio descriptivo de la cohorte de ELT del adulto. Diseñado para dar respuesta al Objetivo 1. Parte 2.- Estudio retrospectivo caso-control anidado en una cohorte sobre factores predictores de refractariedad en ELT del adulto. Diseñado para responder a los Objetivos 2 y 3. VARIABLES: Variable dependiente: Refractariedad a tratamientos médicos según la definición de la ILAE [Kwan et al, 2010] Variables independientes: 16 variables independientes (predictoras) que se dividen en 11 variables clínicas y 5 variables de pruebas complementarias. CONCLUSIONES: " La ELT es un grupo heterogéneo de epilepsias que comparte la misma zona epileptogénica, pero que son diferentes en edad de debut, etiología, neurofisiología, neuropsicología y respuesta terapéutica. " El subgrupo etiológico de Epilepsias Mediales de LT con Esclerosis Hipocampal (EMLT-EH) tiene una edad de debut precoz, se asocia con el antecedente de convulsiones febriles (CF), y su frecuencia relativa depende del tiempo de evolución. Estos datos clínicos apoyan la idea de que sea consecuencia de un proceso fisiopatológico progresivo. " El subgrupo de ELT de causa desconocida incluye a pacientes pertenecientes a los otros dos subgrupos pero en los que las técnicas de neuroimagen no han sido capaces de identificar la lesión estructural. " El subgrupo de ELT de origen estructural está integrado fundamentalmente por Tumores de estirpe glial, Trastornos del desarrollo cortical, Malformaciones Vasculares y Áreas de encefalomalacia isquémica o postraumática. " La expresión clínica predominante en más del 90% de pacientes con ELT son las Crisis tipo I.B. en la clasificación de la ILAE. " 3/4 de los pacientes presentan conducta motora automática durante las crisis. Este porcentaje tendió a ser mayor en el subgrupo de EMTL-EH. " Casi dos tercios de los pacientes con ELT presentan algún tipo de anomalía en el EEG intercrítico. " La ELT acarrea una importante afectación neuropsicológica. Los pacientes con ELT izquierda o bilateral presentan tanto afectación de memoria verbal como visual de los test de fluencia, mientas que los pacientes con ELT derecha tienen tendencia a preservar las habilidades lingüísticas, tanto de memoria como de fluencia. " Existen 14 variables clínicas y de pruebas complementarias asociadas significativamente al desarrollo de refractariedad y 1 que actúa como factor protector. 5 Variables son independientes para el desarrollo de refractariedad: Presencia de automatismos de cualquier clase, RM patológica y EEG interictal seriado patológico, necesidad de más de 3 FAE y necesidad de cirugía. " Es posible establecer un modelo de predicción clínica de evolución a refractariedad. " La presencia de automatismos durante las crisis asociado a una anomalía estructura en RM con protocolo específico de epilepsia y a una alteración focal en EEGs intercríticos seriados se asocia con una probabilidad cercana al 85% al desarrollo de refractariedad asumiendo una prevalencia pretest del 50%.
