Estimates indicate that as many as 1 in 4 Americans will experience a mental health problem or will misuse alcohol or drugs in their lifetimes. These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

Comprehensive and state of the art, the second edition of Pharmacotherapy of Depression offers major revisions of every chapter and the addition of new chapters by expert contributors. The first chapter reviews the neurobiology of depression, which lays the groundwork for understanding the mechanisms of action of antidepressants. In the next chapter, a review of the general principles guiding the diagnosis and medication treatment of unipolar depression is provided. The clinical pharmacology of antidepressants is reviewed in some detail, supplemented by tables that provide information on dosing, indications, and metabolism. Augmentation strategies are reviewed, including the use of non-traditional agents. The chapters that follow next address the use of antidepressants in special populations, such as the elderly and depressed individuals with psychosis, bipolar disorder, substance abuse, and post traumatic stress disorder. The complex issues involving the diagnosis and treatment of depression during pregnancy is thoroughly reviewed in Chapter 8 and provides a synthesis of the scientific literature in the area, one that is noted for contradictory and controversial findings, as well as guidelines for prescribing. The next chapter then provides an overview of the treatment of depression in the pediatric population, highlighting clinical concerns such as suicide risk. The book concludes with two chapters at the interface of medicine and psychiatry in the treatment of mood disorders: managing depression in primary care settings and depression associated with medical illnesses. The outstanding clinician-scientists who have contributed to this volume are all leaders in their fields and represent a broad spectrum of renowned institutions. A timely contribution to the literature, The Pharmacotherapy of Depression, Second Edition, offers busy clinicians from many disciplines a strong scientific foundation that seamlessly transitions into practical recommendations for clinical practice. Working with older adults no longer means working exclusively with frail or disabled people. Older adults are healthier now on average than in decades past, but they still require specialized care. Mental health providers are seeing a growing number of older patients in their practice and may have little experience in the best methods for working with them. To fill that gap, Patricia A. Aren assembled this volume of best practices in treating mental disorders in late life. It includes an overview of geropsychology and the training resources available to help clinicians develop the competencies they need to work with older adults. Chapters focus on evidence-based treatments for late-life depression, anxiety, trauma, and substance abuse disorders, including cognitive behavioral therapy, problem solving treatment, behavioral activation, interpersonal therapy, relaxation training, exposure therapy, substance abuse relapse prevention, and motivational interviewing. Detailed case examples in each chapter illustrate the interventions in action. Although mental disorders are not as common in later life as they are in younger populations, they can be disabling and costly. With the accumulation of evidence over the past twenty-five years, assumptions about whether older adults can benefit from psychotherapy have changed greatly. Not only is psychotherapy a more effective treatment option than medication for many older adults, the effects are as good as those seen in younger adults. This book will help mental health providers take advantage of the latest research and be more effective in their work with older adults.

The prevalence of substance abuse in the severely mentally ill is higher than that in the general population, making this a serious issue for clinicians. Integrated treatment, although the most widely adopted approach, is subject to tremendous variation in its operationalisation, especially throughout different parts of the world. Substance Misuse in Psychosis presents the latest international developments and practical treatment interventions that can be used with co-morbid individuals and their families. Different social and cultural contexts are described and contrasted, along with treatment approaches that have been tailored to address the needs of the severely mentally ill. A final section considers sub-groups, e.g. the young, the homeless, outlining the special issues that need to be considered when providing services for these groups.
