In the United States, TB and HIV are widespread epidemics. The highest prevalence of both diseases remains concentrated in urban centers. While the total number of TB cases reported in the U.S. decreased by 6.4 percent from 1994 to 1995, cases increased among foreign-born persons. Since 1989, TB screening for all people infected with HIV has been the recommended standard of care. The Federal Government combats TB and HIV with direct assistance and financial support to State and local governments. The Health Resources and Services Administration (HRSA), and the Centers for Disease Control and Prevention (CDC) administer separate Federal funding streams for HIV and TB respectively. Through HRSA's Title I programs and CDC's Division of TB Elimination programs, more than 230 separate funding agreements exist between the Federal Government and States, territories, and cities. Clearly, coordination and collaboration among HIV/AIDS programs and TB prevention and control programs at the local, State, and Federal levels are essential if there is to be an effective response to the increase in HIV-related TB. Yet, the extent to which these programs are working together to improve, expand, and coordinate services-and thus decrease the occurrence of TB-is largely unknown. HRSA initiated and funded this study to better understand relationships between HIV/ AIDS and TB programs in Title I eligible metropolitan areas.

AIDS and tuberculosis (TB) have complex health and societal determinants with large impacts. World-wide, the burden of these infectious diseases are huge, especially in the 22 countries with a high TB burden, where more than 80% of TB world-wide occurs and in sub-Saharan Africa, where 67% of the world's HIV/AIDS cases occurred. Despite different ways of transmission and the fact that several separate national programs often exist, tuberculosis and HIV/AIDS are linked. They are often affecting similar population groups (e.g. vulnerable and other minorities groups) and HIV is the most potent risk factors in increasing the risk of progression from latent TB infection (LTBI) to disease among the two billions individuals estimated with LTBI world-wide, while the relative risk of HIV is over 20. Therefore, it is crucial to combine and co-ordinate the efforts when addressing TB and HIV issues. Diverse responses have been developed, both at international and country levels for tackling the different health and social components of these diseases.

This 2011 update of Guidelines for the programmatic management of drug-resistant tuberculosis is intended as a tool for use by public health professionals working in response to the Sixty-second World Health Assembly's resolution on prevention and control of multidrug-resistant tuberculosis and extensively drug-resistant tuberculosis. Resolution WHA62.15, adopted in 2009, calls on Member States to develop a comprehensive framework for the management and care of patients with drug-resistant TB. The recommendations contained in these guidelines address the most topical questions concerning the programmatic management of drug-resistant TB: case-finding, multidrug resistance, treatment regimens, monitoring the response to treatment, and selecting models of care. The guidelines primarily target staff and medical practitioners working in TB treatment and control, and partners and organizations providing technical and financial support for care of drug-resistant TB in settings where resources are limited.
