The development of cancer is a slow process and it very often takes a number of years before a tumor becomes clinically evident. No current te- nique is sensitive and specific enough to detect tumors at their earliest stage, i. e. , when the tumor is smaller than one billion cells. Consequently, the d- ease is usually diagnosed in an advanced state, very often when it is already beyond the reach of therapeutic strategies. This is the main stumbling block to the secondary prevention that would reduce cancer mortality. There is hope, however, because in the last decade we have witnessed an explosion of reports dealing with tumor markers. In many instances, simple, noninvasive diagnostic tests are becoming available to detect the early signs ofneoplasia. Interest in early detection of neoplasia is growing among those basic scientists, cli- cians, and health professionals who realize that progress in reducing cancer mortality is dependent to a great degree on its early detection and prevention. Some disappointment usually accompanies these discoveries, because what at first glance proves to be promising, is in many instances, applicable only to a limited number of cases. What is the cause of these failures? The answer may be that cancer is a multitude of neoplastic diseases in which endogenous and exogenous etiologic factors contribute either sim- taneously or over a lifetime to the development of disease.

Biomarkers are molecular indicators of a biological status and, as biochemical species, can be interrogated to evaluate disease status and therapeutic interventions. Biomarkers may be detectable in the blood, other body fluids, or tissues. The expectation is that the level of an informative biomarker is related to the specific type of disease present in the body. Hence, disease-relevant biomarkers can be used to measure the presence, progress, or intensity of disease. Through a variety of mechanisms, cancer cells provide the biomarker material for their own detection. Tumor biomarkers include cancer-specific mutations or changes in gene expression, both of which can result in aberrant protein expression. These variant or abundant proteins can be detectable in the circulation as the free proteins or as novel autoantibodies to those proteins, the latter indicating that the immune system can provide an exquisitely sensitive sensor of disease. Because cancer cells shed DNA in the circu- tion, an event rarely seen in healthy individuals, tumor-specific genetic changes, such as promoter methylation or gene mutations, are detectable in DNA prepared from plasma or other body fluids. Cancer-related biochemical changes often effect measurable me- bolic variations within a cell or organism. In addition, these biochemical changes result in posttranslational modification of proteins via glycosylation or phosphorylation providing a plethora of opportunity for biomarker discovery.

In Volume I, Analysis of Cells and Tissues, we presented a range of protocols aimed at mapping and analyzing the expression of various molecules of pot- tial interest in metastasis research and for examining their production at the genetic level. In this second volume of metastasis research protocols, we move to the level of living cells and tissues and present methodologies applicable to examining metastatic behavior in vitro and in whole animal models. The methods described in the first section of this volume concentrate on the separation of cell lines with high and low metastatic potential, including the genetic modification of cell lines. The assay systems to test defined aspects of the metastatic cascade are then described in Part II and include cell migration assays, assays for matrix degrading enzymes, basement membrane degrading assays, adhesion assays, and assays of angiogenesis. The role of the specific elements of the metastatic cascade assayed in each of these systems in turn must of course be put into perspective relative to their roles in entire living organisms.
