The growth of uro-oncology has been one of the dramatic features of the past decade in urology. Basic science research into the immunol ogy and molecular biology of cancer is beginning to impact on clinical urology and this poses a number of problems, both for practising urologists and basic scientists. If they are going to contribute to future research in this area they will have to communicate with each other: at present there is a gap which is widening at an increasingly rapid rate. One of the aims of this book is to bridge this gap by collating the views of a group of internationally distinguished scientists and clinicians in order to make this information more easily accessible. It is intended that the book will be useful to any urologist interested in recent developments in uro-oncology both at the clinical and basic science level. I believe the book has a useful function not otherwise fulfilled by present texts. The first section of the book is directed towards bladder cancer.

This volume is a report of the proceedings of the Second International Urological Course held in the Ettore Majorana Centre in Erice, Sicily from the 4-8 November 1978. The meeting was sponsored by the Urological Group of the EORTC (European Organisation for Research in the Treatment of Cancer), the WHO Collaborating Centre for Bladder Cancer, the European Urological Association, the Italian Research Council (CNR) , Italian League against Tumours, Italian Government and Regional Sicilian Government. Contributions were accepted on the understanding that the editors could make certain changes leading towards a uniformity of style but accepting as a priority the importance of early publication, if necessary at the expense of stylistic perfection. Editorial notes have been inserted at infrequent intervals, sometimes to summarise discussions and at others in an attempt to clarify certain issues or to highlight conflicting opinions. Although the work has been divided amongst the editors and co editors it is only proper to acknowledge that the major role in collating and in correcting much of the material has fallen on Mrs MAske who has also typed the manuscript. We should also like to acknowledge the kindness of the Yorkshire Regional Cancer Organisation who allowed Mrs Aske to devote much of her time to this work, Miss S Barrowby who has also given considerable secretarial assistance and the Department of Medical Photography, st James's University Hospital, Leeds whose staff are responsible for many of the illustrations.

Chemotherapy for malignant disease has brought about many rapid and often spectacular improvements in the survival rate of some groups of patients. Yet enthusiasm for these successes has, in part, been offset by the problems in evaluating responses to treatment and by the disappointment of failing to check the progress of still so many other tumours. These reactions will be no surprise to the medical historian but perhaps the expectations from scientific progress nowadays sometimes demand more than can reasonably be expected. Another expectation is that any review of chemotherapy is com pletely up to date, even prophetic. Such is the expansion of the subject that new drugs and trial results are continually being reported but it is this very mass of information that creates its own problems and makes many clinicians despair of finding a balanced judgement on all of this information. This was the challenge accepted by Professor Spiers. He then gathered together a group of colleagues who are amongst the acknowledged leaders in the field of chemotherapy for urological tumours, all of whom have made important contributions to this subject.
