La question de l'autisme met aujourd'hui en évidence, dans le lien social, ainsi que dans les rapports que la science entretient avec la réalité, un réel absolument opaque, impossible de comptabiliser et d'évaluer. L'autiste est un sujet qui refuse le lien à l'Autre à travers la parole, mais qui n'est pas pour autant en dehors du champ du langage. C'est un sujet hors-discours — discours en tant que fondement du lien social —, mais pas hors-langage. L'autisme représenterait ainsi la manifestation clinique la plus frappante du réel, soit ce qui n'est pas pris dans un discours, mais qui cependant opère dans le noyau même de la structure. Cet ouvrage se propose d'aborder la question de l'autisme et la psychose infantile à partir d'une approche structurale dont les fondements sont issus du premier enseignement de Jacques Lacan. Dans ce contexte, l'autisme est conçu comme une position subjective, relevant d'un choix inconscient qui met en jeu l'insondable décision de l'être, selon l'expression de Lacan. Bien que l'autisme ait longtemps été considéré comme une psychose, l'accent est mis aujourd'hui sur les éléments de clinique différentielle : immuabilité, absence de délire, d'hallucinations et de déclenchement, et surtout évolution de l'autisme vers l'autisme — du syndrome de Kanner au syndrome d'Asperger. Une distinction sera ainsi envisagée à partir du mode d'apparition du tableau clinique, érigé comme critère différentiel depuis la description princeps de Léo Kanner en 1943.
