Introduction Chondral and osteochondral defects are often associated with significant pain and disability. Osteochondral allograft (OCA) transplantation can functionally restore large articular defects in the knee. Studies have reported 10- to 15-year survivorship between 71% and 85%, and functional outcomes after OCA transplantation of the femoral condyle have been good to excellent, with 88% of patients returning to sport. While bipolar OCA transplantations for partial and whole joint resurfacing (u201cBioJointsu201d) have resulted in successful outcomes, these surgeries have traditionally had less favorable results than single-surface transplants. The factors reported to consistently influence success for BioJoints have been reported to relate to donor chondrocyte viability, donor allograft bone integration, implantation technique, patient co-morbidities, and patient demographics (e.g., BMI, tobacco use). With the advent of technologies and strategies to address these limiting factors, a comprehensive approach to optimizing outcomes for unicompartmental bipolar biologic joint resurfacing has been employed and assessed. To delineate the mechanistic components for success and complete a head-to-head comparison of BioJoint versus typical standard-of-care non-operative treatment, this study was designed to use a large animal model to test the hypothesis that unicompartmental bipolar osteochondral and meniscal allograft transplantation would be as or more effective for treatment of medial compartment gonarthrosis based on functional, diagnostic imaging, gross and histologic assessments. MethodsWith IACUC approval, medial compartment osteoarthritis (gonarthrosis) was induced in one knee of skeletally mature purpose-bred research hounds (n=8, weighing > 20 kg) using a validated arthroscopic meniscal release (MR) model. Three months after MR, the presence of OA in the operated knees was documented radiographically and arthroscopically, and pre-treatment assessments were performed. Three months after induction of OA, dogs were randomly assigned to one of two treatment groups:u2022tNSAID Control group (n=4) dogs received no surgical intervention (arthroscopic assessment only) and were managed with daily NSAIDs (Carprofen 4.4 mg/kg po q24h) for the study duration.u2022tBioJoint (n=4) received MOPSSM-preserved tissue allografts. Complete distal femur and proximal tibia allografts were obtained from canine cadavers euthanatized for reasons unrelated to this study. Allografts were preserved for 30 days using MOPSSM, and were then implanted into the medial femoral condyle and medial tibial plateau (unicompartmental BioJoint) of the MR knee in each dog using custom-cut techniques and instrumentation. The allografts effectively replaced the entire medial femorotibial joint of each dog in the BioJoint group. Dogs were maintained for 6 months after implantation with clinical assessments (lameness scoring, forcemat kinetics, range of motion, and VAS pain scores), radiographic assessment, and arthroscopic assessments at 1, 3 and 6 months after surgery. Dogs were humanely euthanatized 6 months after surgery and the knees evaluated by OARSI whole joint histologic assessments and cell viability, biochemical content and biomechanical properties of grafts. Treatments were compared for statistically significant (p

Much research has focused on the basic cellular and molecular biological aspects of stem cells. Much of this research has been fueled by their potential for use in regenerative medicine applications, which has in turn spurred growing numbers of translational and clinical studies. However, more work is needed if the potential is to be realized for improvement of the lives and well-being of patients with numerous diseases and conditions. This book series 'Cell Biology and Translational Medicine (CBTMED)' as part of Springer Nature’s longstanding and very successful Advances in Experimental Medicine and Biology book series, has the goal to accelerate advances by timely information exchange. Emerging areas of regenerative medicine and translational aspects of stem cells are covered in each volume. Outstanding researchers are recruited to highlight developments and remaining challenges in both the basic research and clinical arenas. This current book is the 17th volume of a continuing series.

Patellofemoral Pain Instability And Arthritis written by David Dejour and has been published by Springer Nature this book supported file pdf, txt, epub, kindle and other format this book has been release on 2020-05-23 with Medical categories.

This excellently illustrated book adopts an evidence-based approach to evaluate the efficacy of different techniques for the imaging and treatment of patellofemoral pain, instability, and arthritis. The aim is to equip practitioners with an informative guide that will help them to manage disorders of the patellofemoral joint by casting light on the many issues on which a consensus has been lacking. The opening chapters supply essential background information and explain the role of various imaging modalities, including radiography, CT, MRI, and bone scan. The various conservative and surgical treatment approaches for each of the three presentations – pain, instability, and arthritis – are then described and assessed in depth, with precise guidance on indications and technique. Postoperative management and options in the event of failed surgery are also evaluated. Throughout, careful attention is paid to the literature in an attempt to establish the level of evidence for each imaging and treatment method. The new edition has been thoroughly updated, with inclusion of additional chapters, in order to present the latest knowledge on biomechanics, diagnosis, surgical techniques, and rehabilitation.

This book presents the latest knowledge on all aspects of osteoarthritis of the knee. Beyond offering a thorough evidence-based review of the available treatment options, it provides helpful information on such fundamental aspects as anatomy, biomechanics, biochemistry, etiology, pathogenesis, and radiologic assessment. The treatment-oriented chapters cover non-pharmacologic treatment, drug treatment, intra-articular drug and/or cell-based injection therapy, arthroscopic treatment, osteotomy, and joint replacement surgery. The goal is to equip the reader with a sound understanding of both the condition itself and the appropriate treatment strategy in different situations. The importance of taking into account factors such as the degree of arthritis, patient activity, lifestyle, and pain when formulating that strategy is emphasized. The fact that the book extends well beyond the description of surgical treatments means that it will be an excellent source of information and guidance for general clinicians as well as for those who specialize in the management of musculoskeletal disorders.

This book offers a state-of-the-art, evidence-based reference to all aspects of veterinary cytology. Truly multidisciplinary in its approach, chapters are written by experts in fields ranging from clinical pathology to internal medicine, surgery, ophthalmology, and dermatology, drawing the various specialties together to create a comprehensive picture of cytology's role in diagnosis and treatment of animal disease. Firmly grounded in the primary literature, the book focuses on companion animals, with special chapters for species with fewer publications. Chapters are logically organized by body system, with additional chapters on tumors of particular import and diagnostic decision making. The first two sections of Veterinary Cytology focus on cytology techniques, quality control, and special laboratory techniques. Subsequent sections are organ/tissue-based and reflect what is known about the canine, feline, and equine species. This is followed by chapters on non-traditional species, including exotic companion mammals, rabbits, cattle, camelids, non-human primates, reptiles and birds, amphibians, fish, invertebrates, and sheep and goats. The last section highlights some unique features of the applications of cytology in industry settings. Provides a gold-standard reference to data-driven information about cytologic analysis in companion animal species Brings together authors from a wide range of specialties to present a thorough survey of cytology's use in veterinary medicine Offers broader species coverage and greater depth than any cytology reference currently available Veterinary Cytology is an essential resource for clinical and anatomic pathologists and any specialist in areas using cytology, including veterinary oncologists, criticalists, surgeons, ophthalmologists, dermatologists, and internists.
