Das Buch gliedert sich in zwei große Bereiche: Der theoretischen Teil definiert den Begriff der Unterrichtsstörung und beschreibt die gängigen Maßnahmen zur Störungsvorbeugung bzw. Störungsbehandlung und untergliedert sie näher. Schon hierbei zeigen sich große Unterschiede hinsichtlich Ansatz, Wirksamkeit und Aufwand. So kann Störungsprävention auf der Unterrichtsebene, der Beziehungsebene oder der Organisationsebene erfolgen. Bezüglich der Störungsintervention betrachtet das Buch lehrerzentrierte, kooperative und organisatorische Konzepte als Reaktion auf Unterrichtsstörungen. Der zweite Teil geht der Frage nach, inwiefern die theoretischen Überlegungen und Interventionskonzepte tatsächlich in der Schule Anwendung finden. Zu diesem Zweck beobachtete der Autor gezielt Unterrichtssequenzen und interviewte Lehrer und Schüler zu diesem Thema. Gestützt von zentralen Leitfragen veranschaulicht das Buch unter anderem, dass viele Interventionsmaßnahmen theoretisch durchaus als sinnvoll erscheinen, im Schulalltag jedoch aufgrund unterschiedlichster Faktoren nicht oder nur unzureichend umgesetzt werden.
