Computer technology has impacted the practice of medicine in dramatic ways. Imaging techniques provide noninvasive tools which alter the diag nostic process. Sophisticated monitoring equipment presents new levels of detail for both patient management and research. In most of these high technology applications, the computer is embedded in the device; its presence is transparent to the user. There is also a growing number of applications in which the health care provider directly interacts with a computer. In many cases, these applica tions are limited to administrative functions, e.g., office practice manage ment, location of hospital patients, appointments, and scheduling. Nev ertheless, there also are instances of patient care functions such as results reporting, decision support, surveillance, and reminders. This series, Computers and Medicine, focuses upon the direct use of information systems as it relates to the medical community. After twenty five years of experimentation and experience, there are many tested ap plications which can be implemented economically using the current gen eration of computers. Moreover, the falling cost of computers suggests that there will be even more extensive use in the near future. Yet there is a gap between current practice and the state-of-the-art.

This book is a collection of invited contributions, each reflec ting an area of medicine in which computing techniques have been successfully applied; but why the title? From a control system point of view the aim of clinical medicine is to recognise the deviation of a patient from the space of normality, and to propel and steer the patient along a trajectory back to that space. Acquiring and main taining the knowledge and skills of this process is the function of medicine. The first chapter expands on this view. Subsequent chapters written by experts in their respective areas cover a fair range of application. All give considerable insight as to the ways in which the control system approach, facilitated by computational tools, can be of value when applied to clinical problems. The idea for this book arose naturally out of a symposium held at the University of Sussex, Brighton, England, on "Control System Concepts and Approaches in Clinical Medicine" in April, 1982, spon sored by the Institute of Measurement and Control and co-sponsored by the Institution of Electrical Engineers and the Royal Society of Medicine. It is not, however, a "proceedings" of this meeting but rather a collection of essays that reflect developing areas in which many have particular interest. We think the volume is timely and hope that the work described will be an encouragement for others.

The book reports on the current state on HCI in biomedicine and health care, focusing on the role of human factors, patient safety well as methodological underpinnings of HCI theories and its application for biomedical informatics. Theories, models and frameworks for human-computer interaction (HCI) have been recognized as key contributors for the design, development and use of computer-based systems. In the clinical domain, key themes that litter the research landscape of health information technology (HIT) are usability, decision support and clinical workflow – all of which are affected directly or indirectly by the nature of HCI. While the implications of HCI principles for the design of HIT are acknowledged, the adoption of the tools and techniques among clinicians, informatics researchers and developers of HIT are limited. There is a general consensus that HIT has not realized its potential as a tool to facilitate clinical decision-making, the coordination of care and improves patient safety. Embracing sound principles of iterative design can yield significant dividends. It can also enhance practitioner’s abilities to meet “meaningful use” requirements. The purpose of the book is two-fold: to address key gaps on the applicability of theories, models and evaluation frameworks of HCI and human factors for research in biomedical informatics. It highlights the state of the art, drawing from the current research in HCI. Second, it also serves as a graduate level textbook highlighting key topics in HCI relevant for biomedical informatics, computer science and social science students working in the healthcare domain. For instructional purposes, the book provides additional information and a set of questions for interactive class discussion for each section. The purpose of these questions is to encourage students to apply the learned concepts to real world healthcare problems.

This book is the result of several years of enthusiastic planning and effort. Much of this enthusiasm came from the experience of devel{)ping Critical Care Consultant, a large BASIC program for critical care applications (St. Louis, C. V. Mosby, 1985). Working with clinicians showed me that many were interested in learning about clinical applications of computers (and even programming in small doses) but were faced with a paucity of clinical application software. Few had the time or training to develop any such software on their own. After a search through the existing medical literature unearthed relatively little in the way of usable programs, I decided that a series of small clinical applications programs would be of use to the medical community. At the onset a number of strategic decisions were made: (1) the programs would be written in BASIC, in view of its universal popularity, (2) the units used for clinical laboratory tests would be those in common use in the United States, (3) the programs would be simple and easily understood and employ no exotic tricks that were not easily transported across computers, (4) references to the literature would be provided to allow the clinician to critically assess the algorithm or method used himself or to follow up on subsequent criticisms that may have been published, and (5) the programs would demonstrate reasonable standards of software engineering in terms of clarity, trans portability, documentation, and ease of modification.
