Biological markers (biomarkers) are useful tools for understanding the nature and extent of human exposure and risk from environmental toxicants. Biomarkers are classified into three basic categories: exposure, effect, or susceptibility. A marker of exposure is the product of the interaction between a target cell or molecule and a foreign substance (NAS, 1989). These markers can be used to determine the biologically effective dose necessary to elicit a particular physiological change in an organism. A marker of effect is a biochemical or physiological change in an organism that can predict the onset of adverse health effects resulting from a given exposure. Lastly, markers of susceptibility act as indicators of an inherent or acquired tendency of an organism to experience an adverse health effect (NAS, 1989). These markers are already used to detect a variety of diseases and show great promise for developing a better understanding of the mechanicisms of disease. Additionally, biomarkers can be used to establish a more rational basis for quantitative risk extrapolation between species, as weIl as to obtain more precise estimates of the time of critical exposure. These markers can also prove helpful in identifying potentially damaging exposures before the onset of adverse health effects. Biomarkers serve as a valuable exposure assessment tool because they take into account exposure from all routes and integrate exposure from all sources. The book contains the contributions at the NATO Study Institute on Exposure and Risk Assessment of Chemical Pollution – Contemporary Methodology, which took place in Sofia – Borovetz, Bulgaria, July 1–10, 2008. Rapid advances in mathematics, computer science and molecular biology and chemistry have lead to the development in of a new branch of toxicology called Computational Toxicology. This emerging field is addressing the estimation and prediction of exposure risk and effects of chemicals based on experimental data, measured concentration and biological mechanisms and computational models of biological systems. Mathematical models are also being used to predict the fate and transport of substances in the environment. Because this area is still in its infancy, there has been limited application from governmental agencies to regulating controllable processes, such as registration of new chemicals, determination of estimated exposure and risk based limits and maximum acceptable concentrations in different compartments of the environment – ambient air, waters, soil and food products. However, this is soon to change as the ability to collect, analyze and interpret the required information is becoming increasingly more efficient and cost effective. Full implementation of the new processes have to involve education on both part of the experimentalists who are generating the data and the models, and the risk assessors who will use them to better protect human health and the environment.

Advances in epilepsy in recent decades have allowed for improved algorithms for diagnosis and a common understanding of terminology with the development of the International Classifications of Seizures and the Epilepsies. Nevertheless, no common system exists for the estimation of epilepsy severity or its impact on quality of life. Therefore, epileptologists lack the ability to make quantitative assessments of individual patients for comparison of care or for meta-analyses in clinical trials. This book on the Quantitative Assessment of Epilepsy Care approaches this omission by addressing the potential application of clinimetrics within the framework of epilepsy treatment. Clinimetrics is a fast growing discipline concerned with the quantification of clinical symptoms with respect to decision making relating to diagnosis, treatment, and prognosis. These methods allow for the development and validation of clinical scoring systems. For example, the Glasgow Coma Scale is widely used. As a chronic disorder, epilepsy would benefit from clinimetric methodology to create uniformity and to allow for comparisons among evaluations. In addition, epileptologists have not yet developed assessments of health related quality of life to define the overall condition of the chronic epilepsy patient and various therapeutic endpoints. While clini metric tools are essential for research, they will also be useful in clinical practice for the care of individual patients by documenting status and changes over time.
