Advances in medical, biomedical and health services research have reduced the level of uncertainty in clinical practice. Clinical practice guidelines (CPGs) complement this progress by establishing standards of care backed by strong scientific evidence. CPGs are statements that include recommendations intended to optimize patient care. These statements are informed by a systematic review of evidence and an assessment of the benefits and costs of alternative care options. Clinical Practice Guidelines We Can Trust examines the current state of clinical practice guidelines and how they can be improved to enhance healthcare quality and patient outcomes. Clinical practice guidelines now are ubiquitous in our healthcare system. The Guidelines International Network (GIN) database currently lists more than 3,700 guidelines from 39 countries. Developing guidelines presents a number of challenges including lack of transparent methodological practices, difficulty reconciling conflicting guidelines, and conflicts of interest. Malaria remains an important cause of illness and death in children and adults in countries in which it is endemic. Malaria control requires an integrated approach including prevention (primarily vector control) and prompt treatment with effective antimalarial agents. Malaria case management consisting of prompt diagnosis and effective treatment remains a vital component of malaria control and elimination strategies. Since the publication of the first edition of the Guidelines for the treatment of malaria in 2006 and the second edition in 2010 all countries in which P. falciparum malaria is endemic have progressively updated their treatment policy from use of ineffective monotherapy to the currently recommended artemisinin-based combination therapies (ACT). This has contributed substantially to current reductions in global morbidity and mortality from malaria. Unfortunately resistance to artemisinins has arisen recently in P. falciparum in South-East Asia which threatens these gains. This third edition of the WHO Guidelines for the treatment of malaria contains updated recommendations based on a firmer evidence base for most antimalarial drugs and in addition include recommendation on the use of drugs to prevent malaria in groups at high risk. The Guidelines provide a framework for designing specific detailed national treatment protocols taking into account local patterns of resistance to antimalarial drugs and health service capacity. It provides recommendations on treatment of uncomplicated and severe malaria in all age groups all endemic areas in special populations and several complex situations. In addition on the use of antimalarial drugs as preventive therapy in healthy people living in malaria-endemic areas who are high risk in order to reduce morbidity and mortality from malaria. The Guidelines are designed primarily for policy-makers in ministries of health who formulate country-specific treatment guidelines. Other groups that may find them useful include health professionals and public health and policy specialists that are partners in health or malaria control and the pharmaceutical industry. The treatment recommendations in the main document are brief; for those who wish to study the evidence base in more detail a series of annexes is provided with references to the appropriate sections of the main document.

This document describes the main elements that World Health Organization (WHO) technical departments should elaborate on when providing guidance on evidence generation to research and development (R&D) specialists who are developing health products to address critical unmet needs in public health. While WHO’s TPPs, preferred product characteristics (PPCs) and target regimen profiles (TRPs) provide high-level information on the desired characteristics of the product, the target population, and the key endpoints for safety and efficacy evaluation, more detailed guidance is needed to enable the development of suitable trial/study designs aimed at producing relevant evidence for WHO’s normative products such as guidelines. Children continue to be exposed to powerful food marketing in settings where they gather (e.g. schools, sports clubs), during children’s typical television viewing times or on children’s television channels, on digital spaces popular with young people, and in magazines targeting children and adolescents. Such food marketing predominantly promotes foods that are high in saturated fatty acids, trans-fatty acids, free sugars and/or sodium (HFSS), and uses a wide variety of marketing strategies that are likely to appeal to children, including celebrity/sports endorsements, promotional characters, product claims, promotion, gifts/incentives, tie-ins, competitions and games. Food marketing has a harmful impact on children’s food choice and their dietary intake. It affects their purchase requests to adults for marketed foods and influences the development of children’s norms about food consumption. This WHO guideline provides Member States with recommendations and implementation considerations on policies to protect all children from the harmful impact of food marketing, based on evidence specific to children and to the context of food marketing.

In this guideline, the World Health Organization (WHO) provides the most current and relevant evidence-based global public health guidance on the initiation of treatment with pharmacological agents for hypertension in adults. The recommendations target adult, non-pregnant patients who were appropriately diagnosed with hypertension and counselled about life-style modifications. The guideline provides new recommendations on the threshold for the initiation of pharmacological treatment for hypertension, as well as recommendations on intervals for follow up, target blood pressure to be achieved for control, and the cadre of health care workers who may initiate treatment.
