Inhaltsangabe:Zusammenfassung: Wachsender politischer Druck und die Veränderung der Krankenhauslandschaft hin zu mehr wirtschaftlicher Verantwortung zwingen die Krankenhäuser, ihre Arbeitsweise effizienter zu gestalten. Workflow-Management, also die rechnergestützte Steuerung von Arbeitsabläufen nach vorheriger Optimierung der Geschäftsprozesse, wird nach den bahnbrechenden Erfolgen in der Industrie auch im Zusammenhang mit dem Einsatz im klinischen Bereich diskutiert. Ziel ist es, das medizinische Personal von überflüssigen und nicht effizienten Tätigkeiten zu befreien und die Durchlaufzeiten der Patienten zu minimieren. In dieser Diplomarbeit soll neben der Darlegung der gesundheitspolitischen und theoretischen Hintergründe, die konkrete Vorgehensweise beim Einführen eines Workflow-Managementsystems an einer Universitätspoliklinik geschildert werden. Dabei geht es auch um die Frage, in welchem Umfang die Workflow-Managementsysteme der heutigen Generation für die speziellen Anforderungen des Krankenhausalltags geeignet sind. Abstract: Due to the recent political decisions german hospitals will share a greater part of economic responsibilities. To meet this challenge the hospitals will strive to improve workflow efficiency. Workflow management, i.e. the analysis and improvement of work processes with computational support may be one way to achieve these goals. As this has been shown to be effective in industrial surroundings, workflow management is been discussed for its possible use in clinical work as well. Here, workflow management aims at a reduction of superfluous, inefficient work, and thus the time the patient spends in the hospital. This paper describes the political and theoretical background for the implementation of a workflow management system as well as its realisation at a day care centre in an university hospital. In addition it will discuss the problems with today's workflow management systems when used in a clinical setting. Inhaltsangabe:Einleitung: In dieser Arbeit wird anhand verschiedener Praxisbeispiele das Potential von WFMS aufgezeigt, es wird jedoch auch auf Gefahren und Risiken in Zusammenhang mit der Einführung eines solchen Systems hingewiesen. Anhand zu definierender Einsatzkriterien soll im Rahmen dieser Diplomarbeit geprüft werden, in welchen medizinischen Bereichen eines Krankenhauses ein nutzbringender Einsatz von Workflow-Management-Systemen (WFMS) möglich ist. Dazu sollen ausgewählte Abläufe eines Krankenhauses (inkl., ihrer Schnittstellen zum administrativen Bereich) analysiert und modelliert werden. Diese Abläufe sollen dann insbesondere hinsichtlich organisatorischer und wirtschaftlicher Effekte beim Einsatz von WFMS beurteilt werden. Im Rahmen einer Fallstudie soll näher auf die Situation im UniversitätsSpital Zürich eingegangen werden. WFMS versprechen für verschiedenste Anwendungsgebiete im Krankenhaus ein Leistungspotential. Als speziell interessant erscheint die gemeinsame Nutzung des Business Process Redesign (BPR)-Konzeptes zusammen mit dem Konzept der WFMS. Da sich die Integration von BPR und WFMS erst am Anfang befindet, wurde als ein Resultat dieser Arbeit ein theoretisches Konzept für ein "Process Change Management entwickelt. Weitere Resultate sind die Dokumentation der Bemühungen um eine Prozessorientierung am UniversitätsSpital Zürich (USZ), die Bestimmung von Faktoren für eine erfolgreiche Einführung von WFMS in Krankenhäusern und die Erkenntnis, dass die momentane Situation der schweizerischen Krankenhäuser eine meist nur suboptimale Nutzung des an sich vorhandenen Potentials bewirkt. Als weiterführende Arbeiten könnte man sich vorstellen, das vorgestellte Krankenhaus-Prozess-Konzept in der Praxis zu verwirklichen, d.h. eine durchgehende Prozessorientierung zusammen mit einem WFMS in einem Krankenhaus einzuführen. Ferner wäre es interessant mehr über eine optimale Integration neuer Ansätze, wie Action Workflow oder Objektorientierte WFMS, in die Krankenhaus-Prozessmodellierung zu untersuchen. Das vorgestellte Krankenhausinformationssystem KISIM der inneren Medizin würde sich hierfür gut als Prototyp eignen, da das USZ sämtliche Rechte besitzt und das Programm einfach erweitert werden kann (In C++ programmiert). Konkret würde es hier darum gehen ein WAPI zu erstellen, durch welches existierende WFMS die Steuerung übernehmen können. German hospitals are increasingly being asked to improve efficiency while striving to enhance the quality of standards that will afford a high level of patient safety and quality in the treatment process. The main objective of this Ph.D. thesis was to analyze whether digital support software used during surgery has a positive effect on selected clinical process and quality ratios in orthopedics. Two retrospective case-control studies of 383 hip joint endoprosthetic implantations and 297 knee joint endoprosthetic implantations were conducted at a German hospital between 2015 and 2020. Statistically significant results were evaluated commercially within the framework of the German diagnosis-related groups. Results show that the use of digitally assisted surgery leads to significantly shorter hospital stays and reduces postoperative complications, which in turn has a direct impact on treatment costs and hospital revenues.
