This book is the product of four years of collaborative work within the framework of the European Science Foundation's Regional and Urban Restructuring in Europe (RURE) programme. With one exception, all of the chapters have been prepared by participants in RURE - the exception being that commissioned from Conti and Enrietti on Fiat and Italy to provide a fuller coverage of changes in the main automobile producing companies and countries of Europe. A - perhaps the - central theme around which the RURE programme was conceived is that the restructuring of the production system lies at the heart of the changing map of Europe. Equally, it continues to be the case that the automobile industry lies at the cutting edge of the search for viable new models of production. Some eighty years ago the automobile industry occupied a pivotal position in the transition from craft to mass production - indeed "Fordism" came to denote not just a particular micro-economic model of production organisation in the factory but a macro-scale model of economic development, characterized by a particular pattern of relations between mass production, mass consumption and national state regulation. From the late 1960s, however, it became increasingly clear that Fordism as a macro-scale model of advanced capitalist development was reaching its limits.

Medicare, the world's single largest health insurance program, covers more than 47 million Americans. Although it is a national program, it adjusts payments to hospitals and health care practitioners according to the geographic location in which they provide service, acknowledging that the cost of doing business varies around the country. Under the adjustment systems, payments in high-cost areas are increased relative to the national average, and payments in low-cost areas are reduced. In July 2010, the Department of Health and Human Services, which oversees Medicare, commissioned the IOM to conduct a two-part study to recommend corrections of inaccuracies and inequities in geographic adjustments to Medicare payments. The first report examined the data sources and methods used to adjust payments, and recommended a number of changes. Geographic Adjustment in Medicare Payment - Phase II:Implications for Access, Quality, and Efficiency applies the first report's recommendations in order to determine their potential effect on Medicare payments to hospitals and clinical practitioners. This report also offers recommendations to improve access to efficient and appropriate levels of care. Geographic Adjustment in Medicare Payment - Phase II:Implications for Access, Quality, and Efficiency expresses the importance of ensuring the availability of a sufficient health care workforce to serve all beneficiaries, regardless of where they live.

This text aims to present and discuss the innovative Volvo Uddevalla plant, comparing it to other plants - Japanese lean ones and others. The starting point for the book is Volvo's dramatic decision to close its Uddevalla and Kalmar plants, and the debate that followed this decision, both in Sweden and abroad. Both plants were pioneers of the possibilities to unite productivity and the good work, but, following the announcement of their closure, researchers and practitioners in the field of industrial organization from many countries asked why they closed, how they compared with other production concepts, and whether we now see an end of an alternative to Japanese lean production.
