During the 7th International Bile Acid meeting held in Cortina d'Ampezzo. , Italy, during March 1982, there were two important Workshops dealing with recent advances in two areas of bile acid metabolism - Serum Bile Acids i,n Health and Disease and The Pathophysiology of the Enterohepatic Circulation. These Workshops complemented the broad range of topics in bile acid metabolism which were presented formally as part of a Postgraduate Course. The proceedings of these formal presentations have already been pub lished by MTP Press under the title Bile Acids in Gastroenterology but in view of the exciting, important, new information which emerged during both Workshops, the organisers of the meeting felt that it would be valuable to record and edit the proceedings of the Work shops, which form the basis of this volume. The advent of new techniques for measurement of total and indi vidual bile acids in serum now means that accurate and reliable 7 PREFACE measurements of bile acids in peripheral circulation are available to all. This has resulted in a veritable explosion of information in the serum bile acid field and, therefore, the appearance of a critical evalu ation of the technical and basic scientific and clinical aspects of measuring serum bile acids is indeed timely.

This book is the proceedings of the 18th International Bile Acid Meeting, held as the Falk Symposium 141 in Stockholm, Sweden, on June 18–19, 2004. The International Bile Acid Meetings have become some of the most important meetings in the bile acid field worldwide since Herbert Falk decided to sponsor these Symposia in biannual sequence in 1970. The 17 International Bile Acid Meetings in the past have been a great stimulus for new ideas and methods as well as the development of therapeutic applications of bile acids. They have always been a forum where basic scientists and clinicians interact. Many novel results of bile acid research were first presented at these meetings. The meeting in Stockholm again attracted a large number of basic and clinical scientists interested in bile acid and biliary research from all over the world. Knowledge about the regulation of bile acid synthesis, bile acid transport and enterohepatic circulation in health and in disease has increased considerably during recent years. Accordingly, bile acid research is alive as ever. A new chapter of bile acid research was opened by the finding that bile acids are ligands of nuclear receptors which regulate synthesis, metabolism and transport of bile acids and steroids. A large part of the book is devoted to these topics. The 18th International Bile Acid Meeting was again a truly interdisciplinary symposium at the highest scientific level. Its participants enjoyed hearing the latest reports on bile acid research in the city where so many seminal studies on bile acids were performed. It is hoped that the readers of this book will share the enjoyment.

This book is the proceedings of the Falk Workshop on `Bile Acids in Hepatobiliary Disease', which took place at the Royal Society of Medicine (RSM) in London, UK, on 29-30 March 1999, and was held in association with the Section of Measurement in Medicine at the RSM. The main interest in bile acid therapy has been recently in cholestatic liver disease. The proceedings of the workshop not only discusses this, but moves on to examine its possible use in alcoholic liver disease, and moves back to re-examine its role in biliary disease. Leading world experts attempt to define its mechanism of action, and the current role of other non-surgical treatments in biliary disease. The physiology and pathogenesis of cholestatic and alcoholic liver disease and cholesterol gallstone disease is also examined.

Since the last International Bile Acid Meeting in The Hague in 2000, bile acid research has continued to flourish and therapeutic use of bile acids has attained a broader role. Bile acid research has been stimulated by the discovery that bile acids specifically bind to receptors of the cell nucleus. Thereby, they regulate the transcription of transport proteins and enzymes involved in synthesis, transport and metabolism of bile acids and maintain cholesterol homeostasis. Knowledge about genetic defects of bile acid transport which may cause cholestasis or intestinal malabsorption has also increased impressively. Therapy of chronic cholestatic liver diseases, especially of primary biliary cirrhosis, with ursodeoxycholic acid represents an important advance in modern hepatology and has stimulated basic and clinical research to unravel the underlying mechanisms of action and to optimize treatment schedules. This book is the proceedings of Falk Symposium 129, the XVII International Bile Acid Meeting held in Freiburg, Germany, May 30-June 1, 2002, which was again dedicated to both basic and clinical aspects of bile acid research with a focus on the role of bile acids in hepatobiliary and intestinal diseases. The latest findings are presented by the leading scientists and clinicians in the field.
