The 2017 Global Hunger Index (GHI) shows long-term progress in reducing hunger in the world. The advances have been uneven, however, with millions of people still experiencing chronic hunger and many places suffering acute food crises and even famine. According to 2017 GHI scores, the level of hunger in the world has decreased by 27 percent from the 2000 level. Of the 119 countries assessed in this year’s report, one falls in the extremely alarming range on the GHI Severity Scale; 7 fall in the alarming range; 44 in the serious range; and 24 in the moderate range. Only 43 countries have scores in the low range. In addition, 9 of the 13 countries that lack sufficient data for calculating 2017 GHI scores still raise significant concern, including Somalia, South Sudan, and Syria. To capture the multidimensional nature of hunger, GHI scores are based on four component indicators—undernourishment, child wasting, child stunting, and child mortality. The 27 percent improvement noted above reflects progress in each of these indicators according to the latest data from 2012–2016 for countries in the GHI.

This book introduces the human right to adequate food and nutrition as evolving concept and identifies two structural "disconnects" fueling food insecurity for a billion people, and disproportionally affecting women, children, and rural food producers: the separation of women’s rights from their right to adequate food and nutrition, and the fragmented attention to food as commodity and the medicalization of nutritional health. Three conditions arising from these disconnects are discussed: structural violence and discrimination frustrating the realization of women’s human rights, as well as their private and public contributions to food and nutrition security for all; many women’s experience of their and their children’s simultaneously independent and intertwined subjectivities during pregnancy and breastfeeding being poorly understood in human rights law and abused by poorly-regulated food and nutrition industry marketing practices; and the neoliberal economic system’s interference both with the autonomy and self-determination of women and their communities and with the strengthening of sustainable diets based on democratically governed local food systems. The book calls for a social movement-led reconceptualization of the right to adequate food toward incorporating gender, women’s rights, and nutrition, based on the food sovereignty framework.

In 2007 and 2008, the world witnessed a dramatic increase in food prices. The global financial crisis that began in 2008 compounded the burden of high food prices, exacerbating the problems of hunger and malnutrition in developing countries. The tandem food price and economic crises struck amidst the massive, chronic problem of hunger and undernutrition in developing countries. National governments and international actors have taken a variety of steps to mitigate the negative effects of increased food prices on particular groups. The recent abrupt increase in food prices, in tandem with the current global economic crisis, threatens progress already made in these areas, and could inhibit future efforts. The Institute of Medicine held a workshop, summarized in this volume, to describe the dynamic technological, agricultural, and economic issues contributing to the food price increases of 2007 and 2008 and their impacts on health and nutrition in resource-poor regions.
