The results of treatment for childhood acute myelogenous leukemia (AML) have improved considerably during the last ten years. This progress was demonstrated by the two consecutive multicenter studies, AML-BFM-78 and -83, in which almost identical extended multi drug regimes of che motherapy were administered for eight weeks and followed up by two years maintenance. The main difference in the second study was the addition of an eight-day intensive in duction course. Due to this new element, the relapse rate was reduced significantly. Another result of the BFM-83 study was the definition of two risk groups on the basis of standardized treatment, which has lead to a risk-adapted treatment strategy in the third ongoing trial, AML-BFM- 87. This improvement was only possible thanks to the coop eration of pediatricians, physicians, radiotherapists, statisti cians, and especially the staff at the hospitals and reference laboratories. Thus, we would like to thank everyone who has been involved in these studies and hope that they will be further encouraged to improve treatment strategies for AML in children. Myelodysplastic syndromes are to the bone marrow what pneumonia is to the lungs; the response of an organ to a variety of etiologic insults like aging, toxic exposure, infections and auto-immunity. Among infectious causes alone, pneumonia could be the result of a variety of possible pathogens including bacterial, viral, tuberculous or fungal agents. Similarly, MDS cannot be treated as a single disease. Attempts to harness the inherent complexity of MDS by devising `classifications' which group the various syndromes as one disease is as misguided as saying that a pneumonia is not infectious because it did not respond to antibiotics. Progress in the field will occur faster when we re-analyze this premise. Therefore, until a clearer picture of the disease emerges it is best to treat each of the MDS syndromes as a separate entity. Having no classification is better than a misleading one. Cancer research has been notable for its periodic cycles of promise and hope, followed by defeat and disappointments. It is not that there is no solution, but that the problem has not been identified precisely. This book is our attempt to define the most crucial questions related to MDS that need to be addressed immediately through logic, analysis and rigorous experimentation. If the emerging problems appear daunting, then instead of being overwhelmed by them, we should follow the advice of the great 20th century thinker Antonio Gramsci, `pessimism of the intellect must be faced with the optimism of will'.

Since the introduction of new anthracycline derivatives and anthrachi none analogues a few years ago, aclacinomycin A (Aclarubicin) has become an established agent for the treatment of hematologic malig nancies. A special symposium was therefore held during the congress of the German Society of Hermatology and Oncology in Hannover in October 1989 to provide an up-to-date overv.iew. Leading experts from the United States, Sweden, and Germany reported on the results being obtained with aclacinomycin A, alone or combined with other agents, in patients with acute leukemias and myelodysplastic syndromes. This book is based on their contributions. As regards single-agent treatment, aclacinomycin A in myelodys plastic syndromes is dealt with, as well as its application in older patients with acute myeloid leukemia. Four contributions are devoted to the use of aclacinomycin A in combination with conventional or intermediate dose cytosine arabinoside or etoposide in patients with relapsed or refractory acute myeloid leukemia. The results reported indicate that aclacinomycin A has substantial activity in the treatment of hematologic malignancies.
