This title was first published in 2001: We are all persons or selves. But what exactly does it mean that we possess an identity and autonomy as persons or selves? This book explores the related problems of self-identity and personal autonomy within the framework of contemporary analytical anthropology, a blend of analytical philosophy of mind and action with moral psychology. Cuypers critically examines the empiricist bundle theory and metaphysical ego theory of self-identity as well as the hierarchical Frankfurt / Dworkin model of personal autonomy. Arguing that all these standard views are found wanting, Cuypers then offers an alternative 'personalist' theory of personal identity, plus an innovative 'moderately heteronomous' theory of autonomy without ever going beyond the analytical frame of reference. This book arises from a two-fold conviction. The first is that autonomy, despite recent critiques about its importance in bioethics and philosophy of medicine, and the traditional resistance of medicine to its "intrusion" into the doctor-patient relation, is a fundamental building block of an individual's identity and mechanisms for dealing with illness, disease, and incapacity. As such it is an essential component in the health care professional's armamentarium employed to bring about healing. Furthennore, it functions in a similar way to assist the health professional in his or her relations to the sick and injured. The second conviction follows from the fITst. Autonomy is far more complex than appears from the philosophical use of the concept. In this conviction we join those who have criticized the over-reliance on autonomy in modem, secular bioethics originating in the United States, but gaining ascendancy in other cultures. This critique relies on appeals to the richer contexts of persons' lives. Elsewhere the contemporary critique of autonomy appears in a variety of alternative ethical models like narrative ethics, casuist ethics, and contextualism. Indeed, postmodern criticism of all bioethics argues that there is no defensible foundation for claims that one ought to respect autonomy or any other principle as a way of ensuring that one is ethical.

This book brings together insights from the enactivist approach in philosophy of mind and existing work on autonomous agency from both philosophy of action and feminist philosophy. It then utilizes this proposed account of autonomous agency to make sense of the impairments in agency that commonly occur in cases of dissociative identity disorder, mood disorders, and psychopathy. While much of the existing philosophical work on autonomy focuses on threats that come from outside the agent, this book addresses how inner conflict, instability of character, or motivational issues can disrupt agency. In the first half of the book, the author conceptualizes what it means to be self-governing and to exercise autonomous agency. In the second half, she investigates the extent to which agents with various forms of mental disorder are capable of exercising autonomy. In her view, many forms of mental disorder involve disruptions to self-governance, so that agents lack sufficient control over their intentional behavior or are unable to formulate and execute coherent action plans. However, this does not mean that they are utterly incapable of autonomous agency; rather, their ability to exercise this capacity is compromised in important respects. Understanding these agential impairments can help to deepen our understanding of what it means to exercise autonomy, and also devise more effective treatments that restore subjects’ agency. In this book, we explore the potential power of the stories that youth construct as one route for such movement. Our emphasis is on how those stories serve to build a sense of identity for youth and how the kinds of stories youth tell are informed by their broader contexts – from parents and friends to nationalities and history. Identity development, and in part- ular narrative identity development, concerns the ways in which adolescents must integrate their past and present and articulate and anticipate their futures (Erikson, 1968). Viewed in this way, identity development is not only unique to adol- cence (and emergent adulthood), but also intimately linked to childhood and to adulthood. The title for this chapter, borrowed from the Joyce Carol Oates story, highlights the precarious position of adolescence in relation to the construction of identity. In this story, the protagonist, poised between childhood and adulthood, navigates a series of encounters with relatively little awareness of either her childhood past or her potential adult futures. Her choices are risky and her future, at the end, looks dark.
