This volume is arecord of the proceedings of a NATO Advanced Study institute on "Biochemical and Bio logical Markers of Neoplastic Transformation" held September 28 - October 8, 1981, at Corfu, Greece. As early as 1860, Rudolf Virchow provided the first genetic concept of cancer by postulating "Omnia ceZZuZa e ceZZuZa ejusdem generis", a modification of the then exisiting cell theory "Omnis ceZZuZa e ceZZuZa". Thus, the idea that all cells originate from the parent cell was extended to the idea that all cancer cells come from the "paren t" cancer cello But how the first cancer cell arose, or in other words, how anormal cell changed to a cancer cell, is, even after 120 years, a mystery. Experimental studies of the past have convinced us that a number of factors contribute to the neoplastic transformation of anormal cell, but our knowledge on the mechanisms involved in this process is still in an embryonic state. In the last few years, however, this field has witnessed a most remarkable advancement cata lyzed by the development of modern technology in the al lied fields of immunology, the production of mono clonal antibodies, molecular biology, and sequencing v PREFACE and cloning of ONC genes. Presently, it is becoming more and more evident to the wishful mind of those engaged in this research that we are approaching a turning point. Thus, an assessment of the present situa tion will be most desirable at this time

Biomarkers are molecular indicators of a biological status and, as biochemical species, can be interrogated to evaluate disease status and therapeutic interventions. Biomarkers may be detectable in the blood, other body fluids, or tissues. The expectation is that the level of an informative biomarker is related to the specific type of disease present in the body. Hence, disease-relevant biomarkers can be used to measure the presence, progress, or intensity of disease. Through a variety of mechanisms, cancer cells provide the biomarker material for their own detection. Tumor biomarkers include cancer-specific mutations or changes in gene expression, both of which can result in aberrant protein expression. These variant or abundant proteins can be detectable in the circulation as the free proteins or as novel autoantibodies to those proteins, the latter indicating that the immune system can provide an exquisitely sensitive sensor of disease. Because cancer cells shed DNA in the circu- tion, an event rarely seen in healthy individuals, tumor-specific genetic changes, such as promoter methylation or gene mutations, are detectable in DNA prepared from plasma or other body fluids. Cancer-related biochemical changes often effect measurable me- bolic variations within a cell or organism. In addition, these biochemical changes result in posttranslational modification of proteins via glycosylation or phosphorylation providing a plethora of opportunity for biomarker discovery.

The success rate for treatment of primary neoplasms has improved sig nificantly due to improved surgical, radiotherapy, and chemotherapy methods, and by supportive patient care. In contrast, the treatment of cancer metastases, the cause of most cancer deaths, has not been very successful. Approximately 50% or more of patients with primary malignant neoplasms already have established metastases. Consequently, the most important problem in cancer treatment is the destruction or prevention of metastases. Metastases research has obvious clinical importance. Yet it has only been recently that investigators have attempted to study the mechanisms in volved in this process. This is in part due to the complexity of metastases formation. A metastatic colony is the result of a complicated series of steps involving mUltiple tumor host interactions. It is expected that multiple biochemical factors and gene products derived both from the host and the tumor cell may be required for the metastasizing tumor cell to invade, survive host defenses, travel in the circulation, arrest and adhere in the target organ, invade out, and grow as a metastatic colony. Some of these factors have recently been identified by investigators who have focused on individual steps in the metastatic process and have employed new technologies in immunology, biochemistry and molecular biology. The purpose of this volume is to capture some of the excitement in the field of metastases based on such new discoveries.
