Diseased heart valves are a growing public health concern, ranging among the most important causes of heart failure and sudden death. This volume reviews the most current information on valvular disease pathophysiology, epidemiology and evaluation as well as management strategies for affected patients. Over the last three decades, major advances in the management of diseases of the valves have been seen. While cardiac catheterization was in the 1970s, the only means for con?rming the diagnosis, Doppler echocardiography has become the corner stone for accurate assessment of valve disease, even in fetuses. Now not only the disease can be identi?ed but also the rate of disease progression (e. g. lea?et calci?cation and stiffness) can be accurately measured and quanti?ed by echocardiography and multislice/electron beam CT imaging. Surgical treatment of valve disease has also w- nessed great advances, having moved from traditional valve replacement to sophisticated repair procedures and the use of extracorporeal pump support in some patients. Robotic valve surgery has also proved a great success, in well selected cases, and is expected to have a fruitful future. Furthermore, in special circumstances, conventional surgical valve excision has now been replaced by percutaneous catheter-based valve replacement for aortic and pulmonary disease. This approach reduces the risk from the surgery itself, avoids general ana- thesia and many potential post-operative complications. This book discusses the practicalities of the diagnosis and treatment of the various aspects of common heart valve d- eases, covering most clinical and surgical issues. Dr Rose honoured me with a request for a Foreword. I am delighted to oblige. The treatment of valvular heart disease has improved remarkably in the past 40 years. Many factors have contributed; not least being the introduction of artificial heart valves for treatment, more than 25 years ago. Their use has shown that they are good, but not an ideal substitute for native valves. A galaxy of pathological changes are associated with the insertion and malfunction of artificial heart valves. Each has to be defined, classified and related to clinical procedures or problems with a prosthesis; then a means sought to prevent them. Often, in understanding their cause, investigative procedures have/will improve patient care and broaden knowledge in other spheres. Dr Rose has been a student of this pathology for many years and has made many contributions. No doubt his interest in the area was whetted by colleagues in Cape Town, leaders in the field of cardiovascular medicine and surgery. This monograph provides a collected review of his experience. In it one finds lessons in geographic pathology, in considering the causes of valvular heart disease in Cape Town and valuable information regarding the identification of artificial heart valves and a means of examining a heart bearing one.
