Why has the response to HIV/AIDS been unique? How did civil society organizations gain access to global decision-making forums to demand exceptional attention and resources for HIV/AIDS? This book seeks to answer these questions, among others, through a critical international relations approach that enquires into the role of civil society in global health governance. It documents how civil society forged the initial response to HIV/AIDS within a rights-based paradigm, and built international networks. It analyses why civil society was able to gain the right to participate in global health institutions and assesses what influence civil society representatives have within these institutions, particularly focusing on outcomes related to institutional legitimacy and downward accountability. It then discusses changes in the broader political economy of global health and how HIV/AIDS organizations have, or have not, adapted to these shifts. Finally the book tells the story of the many struggles civil society organizations have engaged in to advance a rights-based response to HIV/AIDS, the transformations achieved and the resistance experienced.

How resources are being used to fund the community response to human immunodeficiency virus (HIV) and acquired immune deficiency syndrome (AIDS) is of considerable interest to the donor community and governments. In the past decade, international funding for the HIV and AIDS response provided by governments rose from about US$1 billion to US$8.7 billion; donors increasingly shifted their financial support toward funding community responses to this epidemic. Yet little is known about the global magnitude of these resource flows and how funding is allocated among HIV and AIDS activities and services. Although some studies have been carried out to gather information on the community response by civil society organizations (CSOs), most of them provide only partial information limited to a specific intervention (for example, orphan support) or specific local communities. To address this knowledge gap, the report attempts to answer the following questions: How large is donor funding for community-based interventions that are run by either large nongovernmental organizations (NGOs) or smaller community-based organizations (CBOs)? How do the funds reach various types of CSOs? What are CSOs' other sources of funding, and to what extent are the CSOs dependent on donor funding? How are these funds used for by CSOs? Are there differences among different types of CSOs working on HIV and AIDS?

Investing in Communities Achieves Results fills an important gap in the global knowledge on community level results and resources related to HIV and AIDS. While communities, in spite of their limited resources, have played a key role in the HIV/AIDS response, their contributions and innovative approaches to prevention, treatment, care, and support have not always been the focus of systematic and rigorous evaluations. To address this deficit, a series of studies-including evaluations in Burkina Faso, India, Kenya, Lesotho, Nigeria, Senegal, South Africa, and Zimbabwe-were undertaken over a three-year period (early 2009 to early 2012), helping to build a robust pool of evidence on the effects of community-based activities and programs. A unique feature of this multicountry evaluation was the collaboration between two international organizations (the World Bank and the United Kingdom's Department for International Development) and a major civil society network (the U.K. Consortium on AIDS and International Development). Other attributes that contributed to the successful outcome were the sustained consultation process with civil society and stakeholders at the local, national, and global levels, and the collaboration among high-caliber, multi-disciplinary researcher teams. The book's findings are promising. At varying levels, depending on the country context, the HIV response in communities was shown to improve knowledge and behavior and increase the use of health services- and even decrease HIV incidence. Evidence on social transformation was more mixed, with community groups found to be effective only in some settings. Each study in the evaluation provides a partial view of how communities shape the local response; however, taken together they constitute a significant pool of rigorous evidence on the contributions of communities, community groups, and civil society to the national and global HIV and AIDS response. The studies suggest that communities have produced significant results at the local level, which contribute to outcomes at the national level.

This timely book looks critically at the policy response to AIDS and its institutionalization over time. It raises important questions about who benefits, who decides, and in whose interests decisions are made. Taking the early international response to the epidemic as its starting point, and focusing on the work of agencies such as UNAIDS, it identifies two logics underpinning strategy to date. First, the idea of HIV as a ‘global emergency’ which calls for an extraordinary response. Second, the claim that medicine offers the best way of dealing with it. The book also identified the rise of something more dominant – namely Global AIDS – or the logic and system that seeks to displace all others. Promulgated by UNAIDS and its partner agencies, Global AIDS claims to speak the truth on behalf of affected persons and communities everywhere. Founded on solidarity claims concerning the international HIV movement, and distinctive knowledge practices which determine what needs to be done. Alternative views about the nature of the epidemic or the best response are rejected as irrelevant for falling outside the master framing of the epidemic that Global AIDS provides. But to what extent is this biomedical and emergency framing of the epidemic sustainable, and to what extent does it speak to the sustainability of lives as affected people wish them to be lived? Civil Society Organizations (CSOs) can make a vital contribution to public health and health systems but harnessing their potential is complex in a Europe where government-CSO relations vary so profoundly. This study is intended to outline some of the challenges and assist policy-makers in furthering their understanding of the part CSOs can play in tandem and alongside government. To this end it analyses existing evidence and draws on a set of seven thematic chapters and six mini case studies. They examine experiences from Austria Bosnia-Herzegovina Belgium Cyprus Finland Germany Malta the Netherlands Poland the Russian Federation Slovenia Turkey and the European Union and make use of a single assessment framework to understand the diverse contexts in which CSOs operate. The evidence shows that CSOs are ubiquitous varied and beneficial and the topics covered in this study reflect such diversity of aims and means: anti-tobacco advocacy food banks refugee health HIV/AIDS prevention and cure and social partnership. CSOs make a substantial contribution to public health and health systems with regards to policy development service delivery and governance. This includes evidence provision advocacy mobilization consensus building provision of medical services and of services related to the social determinants of health standard setting self-regulation and fostering social partnership. However in order to engage successfully with CSOs governments do need to make use of adequate tools and create contexts conducive to collaboration. To guide policy-makers working with CSOs through such complications and help avoid some potential pitfalls the book outlines a practical framework for such collaboration. This suggests identifying key CSOs in a given area; clarifying why there should be engagement with civil society; being realistic as to what CSOs can or will achieve; and an understanding of how CSOs can be helped to deliver.
